Whole operation looks good. Well is not connected to production lines yet. They are in a testing phase right now.

0584321421 2008-10-01 Preliminary Restoration Violations Noted Preliminary restoration is not done. Pit liner pieces are sticking out of a piles of soil and drill cuttings. The disturbed area is 171 feet by 210 feet. The top soil is in a pile. A soil sample was taken of the disturbed site. I received a call from the land owner Chris Brust on 9-30-08 he wanted to meet with me on location. I had a meeting with Mr. Brust on 10-1-08 to discuss the restoration violations. Mr. Brust is upset and wants take the companys bond. I explained that he cannot take the bond him self. Mr. Brust wants to approve of the contractors before they start the work. Mr. Brust also stated his concern for the access road to the water tower. The road has been recently resurfaced and is in good shape. Well does not have identification.

I also requested a written time line on getting the well in production or plugged. Terry said that they have not had a chance since taking over the wells to evaluate and see if it is capable or needs plugged. Terry told me that Bill Mitton is on vacation and will be back in a week. He will have Bill get in touch with me.
1205956799 2008-02-07 Drill / Deepen / Reopen No Violations I received a complaint from Mr. Brust Re: Site is torn up and the electric is a hazard and one of his cows got killed, also the road to the water tower that was used during drilling is torn up. I went to the site on 2-7-08 and the road was in bad shape with mud and ruts. The Electric service had been inspected by the Ross County Building Dept. I could see nothing wrong with the electric lines. I noticed that the main disconnect for the electric service was turned off. So the system did not pose a threat at this time. All electric lines are buried except at the well head. These lines will be buried during restoration. The well site is in fairly good shape considering the weather and that the company has 9 months to restore the site. I did not see the dead cow. I called Paxton and they are sending a crew out on 2-8-08 to fix the road and to straighten up any other issues. I suggested a fence at the wellhead to keep the cows away from equipment. I contacted Mr. Brust and explained about the restoration time frame and that the electric is turned off. I told him that Paxton would be getting a hold of him to discuss the dead cow.

1447416909 2007-10-28 Drill / Deepen / Reopen No Violations Called out 12:30 arrived on site and found the drill pipe stuck in the hole. Could not go up or down. They had TD at 497 and were coming out of the hole. Cement trucks were on site. Drill mud was to thick and caused the drill pipe to hang up. After circulating and some air bursts the drill pipe came free. Ran 463 of 8 and cemented to surface. Good circulation and cement returns to the surface.
1508303828 2008-10-06 Preliminary Restoration No Violations Meeting on location with Dale Stewart (contractor for Paxton Madison) We went over the scope of the work to be done. Remove the contaminated soil and replace it with good soil from a permitted location. Seed, lime, fertilize and mulch the site. I called Chris Brust (landowner) before the meeting and after the meeting to keep him informed on the progress.

I have contacted Paxton Madison and they are waiting for the ground to dry out. Then they are going to finish both the 20091 and the 20093. At the same time. Plastic pit liner pieces are all over the ground.
1827367071 2010-03-08 Administrative Inspection No Violations Received call from Bill Mitton. 3:20pm Re: will get identification on wells this week. Will attempt to produce and work with the well this spring. Will be bringing a rig down. If not successful will plug the well while rig is in area.
1882745782 2012-08-16 Administrative Inspection No Violations I received a call from Chris Brust (landowner) Chris stated that he wants to know what the colored flagging is on his gate. I told Chris that the well is permitted for plugging and that I had received notification from Knox Energy that they are in the area looking at the wells. Chris said that he thinks the well is leaking some gas. He also stated that he does not want the well plugged. He stated that he wants ownership of the well. I told Chris that is a civil matter between himself and Knox Energy. We discussed the requirements for well owners. I am sending Chris a landowner packet.
