Fire extinguishers in good working order were in the dog house, on the boom, the light plant and mud pump. Hand rails were up. Safety fence was up. BOP was in place. Pits were in good repair. Operator had to drain fresh water tank Sunday and pump pits into tank. Operator also had additional frac tank brought onto site and pumped pits into frac tank. Area received 2.45 inches of rain in 45 minutes. Sunday.

The annulus valve is open to a pop off valve that is plumbed into the production tank. The tank battery and well are in good repair and surrounded by a chain link fence. The tank battery consists of two 100 BBL production tanks, a 50 BBL poly brine tank, and a mechanical separator. The vent stack on the production tank is plumbed into a 55 gallon charcoal filter. The vent stack is then plumbed up and topped with a spark arrestor. The proper identification is located on the tank battery. The lease road is in stable condition.

The well was fenced and locked. Production records for this well were up to date. The tank battery consisted of two 100 BBL production tanks, one 50 BBL poly brine tank and a mechanical separator. Valves on the separator were open and the chart showed production. Tank valves were equipped with flat plugs. The production tank vent stack was plumbed into a 55 gallon charcoal filter and topped with a spark arrestor and had proper lids and hatches. The tank battery was diked and fenced with a locked man door. Proper, legible signage was located at the tank battery and the lease access was stable. No violations were observed.

Start time 15:05 end time 15:20, 20psi loss of pressure chock valve was leaking, not closed all the way. Pressured stabilized after 5 minutes. No lose of pressure at the annulus. Dog house has working fire extinguishers in it as well as the mud pump.

Soil was damp when digging, there was approximately a foot of black topsoil, 4 feet of brown subsoil then into a gray clay. No flow of ground water noted. Pipe was new R-3 20lb pipe, set open ended. Petroset cementing company broke circulation ran 1 bag of cotton seed hauls ahead. Mixed cement and ran cement until it returned to surface. Petroset changed over and displaced cement. 18 BBL return of good cement to surface. Cement held at surface. Berea top was 200 bottom 245. Operator had collected cutting samples every ten feet. Permit was posted in the dog house. Safety fence was up. Pits were in good repair. Lease road was stable. Permit sign posted at the entrance of the lease road.
1276178201 2010-06-09 Drill / Deepen / Reopen No Violations Operator was mixing in 2nd load of cement fines into pits to solidify them. Blow pit is soft and will need a 3rd load to complete solidification, 3rd load has been ordered. Operator was sufficiently mixing pits to solidify them top to bottom. Lease road is stable and in good repair. Annulus pressure was zero. Opened annulus with operator, annulus was on vacuum.

Operator was putting up safety fence around pits. Drainage controls are in good repair. Operator pushed dirt up to the silt fence north end of blow pit. Dirt was up to the silt fence, fence was in place and a berm is behind fence. Locks, pop off and gauge are on annulus.
1899302184 2010-06-02 Drill / Deepen / Reopen No Violations On site for long string job. Operator ran new R-3 11.6 lb pipe. Hole was TD at 3880 pipe set at 3867. Top of the Lockport was 3186. Cementing 100 above Lockport 3086 TD 3880= 794 of cement coverage needed. 794 X .2278 / 1.18= 153 sacks of class A cement needed for the job. Operator ran 210 sacks of class A cement, no circulation issues noted during cementing. Pits were in good repair. Lease road was stable. Operator will replace silt fence and straw bails north end of site. Drainage controls were washed out during flood event. Area had a 250 to 500 year rain event, 2.45 inches of rain in 45 minutes.

Took readings of Clinton gas and annulus gas with operator and gas meter the DMRM is field testing. Clinton gas readings: CH4 9.10, CO 277, H2S 2ppm. Annulus readings Ch4 8.5 H2S 2. No H2S readings around well head, H2S was detected only when probe was inserted directly into the annulus.
