Hole took some water in the Berea. Operator ran 241/ 251; with landing joint of new R-3 20lb pipe, set open ended. Operator broke circulation mixed one bag of cotton seed hauls ahead. Ran cement, shut down to clean rock out of hopper started job over. 6BBL of good cement returned to surface, cement held at surface.
0380299078 2011-03-10 Production Wells No Violations Latch on the man door to the tank battery is broken, latch needs repaired. Lease road gate needs a locking mechanism. Well head is in good repair. Well head is fenced with a cage top. Lease road is stable.
0577310183 2010-07-19 Preliminary Restoration No Violations Site has been rough graded. Tank battery has been installed, two 100BBL tanks and a mechanical separator in the battery, spacing is good. Well is a rabbit well, lock is missing from the well head man door. Spark arrestors and charcoal filters are in place. Operator used board on board fencing. Lease road is stable. Site needs final grading and seeded. Identification sign is at the entrance to the site. PVC drain in the north west corner of the battery valve was open at the time of inspection. valve was closed.
1066618762 2013-06-19 Preliminary Restoration No Violations My inspection found the well to be equipped with a plunger lift unit. The annulus valve was open and plumbed into a pop off valve that was plumbed into the production tank. The well and tank battery are in good repair and surrounded by a wooden fence. The tank battery consists of two 100 BBL production tanks, a 50 BBL poly brine tank, and a mechanical separator. The production tank is vented through a 55 gallon charcoal filter and up into a spark arrestor. The tanks all have sealed lids. The well is properly identified. The lease road is in stable condition and is secured by a locked gate. The site has been graded and vegetation is established.
1156933370 2017-02-14 UrbanProduction Wells No Violations During this inspection, it was observed that the well was equipped with a plunger lift assembly and the valves on the wellhead were open. The well was fenced and locked. The last reported production for this well was submitted in 2015. The tank battery consisted of two 100 BBL production tanks, one 50 BBL poly brine tank and a mechanical separator. The valves on the separator were open and the chart showed production. The tank valves were equipped with flat plugs. The vent stack on the production tank was plumbed up into a 55 gallon charcoal filter, then plumbed into a spark arrestor. The tanks were equipped with the proper, lids and hatches. The tank battery was diked and fenced with a locked man door. The lease access was stable. Signage was not located at the well or tank battery and needs to be installed. I contacted Mark Turpak from Cutter about installing signage at the wellsite.
1263221707 2010-01-09 Drill / Deepen / Reopen No Violations On site for rig move on. 2 to 3 feet of snow around location, location has been cleared of snow. Permit sign posted at the entrance to the site. Permit posted in the dog house. Topsoil has been stockpiled to the south. Drainage swale has been constructed east and south of location, silt fence installed at the toe of the slope. Lease road is rock to the site. Mufflers in place on the rig, mud pump and air plant. Rig is oriented north/south. Fire extinguishers in place and in the green on the rig, boom, mud pump and light plant. Hand rails were being installed at the time of inspection. Soil strata that the pits were constructed in was brown clay with the bottom being stiff blue clay. No ground water, rocks or roots noted. Reminded operator to slope pit sides, side were sloped.
1263326288 2010-01-12 Drill / Deepen / Reopen No Violations Drill depth 2213, drilling on salt water. Operator is going to starch up going into the Newburg, this will add an extra day to drilling. Should be noted for other drillers, Newburg formation in this area swells, starch is needed at times to keep sides of hole clean and prevent drilling in. Pits were in good repair. Fluid level was good. Safety fence was up. BOP was in place. Waived BOP test the day of surface cement. BOP was stable no movement was noted. Minor fuel spill >quart of fuel, on the sub-base of the rig. Safety sings were up at the end of the lease road. Silt fence was in place. Eric Poulson was the driller this day.
1264700366 2010-01-22 Drill / Deepen / Reopen No Violations Pits have been solidified. Few inches of water standing on the surface pit. Pits were firm. Annulus pressure was zero. Safety fence was up around pits and well head. Drainage controls were in place.
1284989066 2010-09-17 Production Wells No Violations Well check due to water well complaint at the Noble residence. On site with Mark, pumper for Cutter Exploration. Annulus pressure was 40psi. Casing record for the well: 11.75 inch pipe set at 84 feet, 8.625 casing set at 251 feet, 83 feet below the base of the Berea Sandstone. Casing shoe pressure allowable .303 X 251= 76psi X .70= 53.23psi. Annulus blew down in under 2 minutes. Information from Warren Schoenfelt geologist for Cutter, 10 day test annulus reached a pressure of 43 psi.
1840050516 2010-01-14 Drill / Deepen / Reopen No Violations Drilling on fluid and starch, 2 hour sticks. Pits are in good repair, edges are snow covered. Small fuel spill under rig, operator started cleaning up during inspection. Per Roy Poulson, spill was an out of body moment by one of his employees. Spill was a gallon or less.
