0093031348 2010-04-07 Production Wells No Violations Walked woods to the east of the well, streams and wetlands were checked for crude oil, no oil was located. Operator has contained any remaining oil on the pond in the north east corner of the pond. Operator heat treated the south edge of the pond. Blue die was added to the pond for the landowner. Operator was repairing and adding additional drain tile for the land owner at the time of inspection. Existing drain tile was still discharging a film of oil, operator is going to flush tile and place a boom at the discharge to prevent any further escape of oil.

0526161425 2010-07-19 Production Wells No Violations Drain pipe has been removed. There is new oil in the dike and absorbent pads have been placed. Water standing in the dike has oil floating on it, there is staining on the rock bottom of the dike. Approximately 1 gallon of oil in the bottom of the dike. There is oil staining on the bottom of the board on board fence east side. Dug test hole at the base of the staining, no quantity of oil noted in the soil. Test hole in the bottom of the dike 6 inches deep, clay is saturated with fluid. Out side of dike, dug test hole 6 inches deep at the base no wicking of fluid noted. Contacted operator and left message to contact me regarding oil in the dike. Spoke too operator oil is from under the tank from leak in the winter. Operator is allowing water to build up to float oil out of rocks. Operator is using a trash pump to pump fluid form the bottom of the dike into the brine tank for disposal. Bull plug in the side of the 100BBl tank had residual weeping coming out of the threads. Battery will need clean. Well head is in good repair. Operator has removed oil debris from around pond that well leaked into.
0656818784 2018-11-02 UrbanProduction Wells No Violations I conducted an inspection on this well. The well was equipped with a plunger lift assembly and in production. The well was fenced with a locked man door and legible signage. The tank battery was comprised of one 100 BBL tank, one 35 BBL tank and a mechanical separator. The diked tank battery was fenced with a locked man door and legible signage. The lease access was stable.

Boom can be removed from pond at this time. Advised operator to run the boom across the east side of the pond before final removal. Few 3 to 3 inch spots and streaks of film floating on the east side of the pond. Dike has been repaired at the tank battery. PVC pipe in the dike has been sealed. The ditch north east of the battery needs to be backfilled. Area around tank battery has been seeded and mulched.
1134314847 2010-04-06 Production Wells No Violations On site in the afternoon. Majority of oil has been removed from the pond. Kurt from the EPA is on site. Drain tile was tested with red die to check direction of flow and discharge point. Drain tile discharges to the east and the west. Red die was in the sump before the pond. It was confirmed that oil in the pond was from the tank battery.
1149124264 2011-03-10 Production Wells No Violations Dike drain was open at the time of inspection. There was diapers adjacent to the 100Bbl tank in the battery. Oil staining was evident on the diapers. There was a minor oil sheen on the water adjacent to the separator and the drain valve, less than a cup of oil. No oil sheen on the water out side of the dike. Drain valve was closed by inspector. Oil leaks noted on some of the fittings at the separator. Pictures taken during inspection. Area has had a snow melt and rains, drainage ditch from the battery to the woods was full of water, water was over the edge of the ditch. E-mail sent to Warren Schoenfelt regarding issues and request that they be corrected.

1270576602 2010-04-05 Production Wells Violations Noted Received call from Chester zoning inspector, they have received a complaint from a Mr. John Groth at 12259 County Line Road Chester Ohio, oil in his pond. An oil leak around the tank battery of this well was noted by the zoning inspector. Arrived on site, there had been a release of oil at the tank battery. Oil staining on the board on board fence was apparent. Brine and oil was ponded out side of the dike north east corner of the battery approximately one barrel. A PVC pipe had been installed in the dike, no caps were on the pipe at the time of inspection. The operator had dug a ditch at the north east of the battery, to drain water away from the battery. Two booms had been placed across the ditch. A white PVC drain tile discharges into the ditch as well. There was crude oil staining in the ditch and on the PVC pipe. Walked drainage ditch along Sherman Road to County line road, no crude oil noted along road ditch. Pond on County line road was located. Crude oil, approximately 1 barrel in volume was on the surface of the pond at 12259 County Line road. Oil was also noted in the drainage ditch that the pond discharges into. Crude oil was stopped 50 feet before the road ditch on County Line road by vegetation. Scott Miller from Cutter Exploration arrived on site. Walked woods and oil well site with Scott attempting to determine how the oil made its way into the pond. Kurt Koller from Ohio EPA arrived on site. Drain tile was located in the ditch the operator dug to drain water away from the tank battery. Drain tile was followed back to the pond, 715 feet north west of the tank battery. John Groth the Land owner arrived on site, John gave approval to start clean up. Land owner wasnt sure if oil had been there a week or a month. Area had recently had a covering of over two feet of snow and ice. Residence is not Mr. Groths primary residence. Operator brought a mini hoe on site and dug up the tile and disconnected it before the pond. Sump was construc
1277403980 2010-06-24 Production Wells Violations Noted Checked pond on the Grath property, oil has been removed. One pad and one pile of debris needs removed. Debris is on the north side of the pond. Land owners driveway has had a new layer of 57 gravel placed on it. Well site, area around the battery has been graded, seeded and vegetation is starting to become established. Operator has re-opened drainage ditch from the dike. Operator has place a valve on the PVC drain pipe in the battery. Water is standing at the discharge of the PVC pipe. Standing water has oil on it. Inside the dike there are two pads at the inlet of the PVC pipe. CL- in the water out side of the dike was 631ppm. Dammed the end of the drainage path with clay, scrapped up straw stained with oil and placed it inside the channel that is dammed up. Man door was locked. Permit sign was up. Called Warren Schonfelt from JR Smail, requested pads be placed out side dike, drain channel to be filled in, debris from around the pond and pad in the pond be removed and pvc pipe in the dike be removed.

The well and tank battery are in good repair and are surrounded by a wooden fence. The tank battery consists of a 100 BBL production tank, a 50 BBL poly brine tank, and a mechanical separator. The proper identification is located on the tank battery. The lease road is in stable condition.

The valves on the separator were open and the chart showed production. The vent stack on the production tank was plumbed into a 5 gallon charcoal filter and topped with a spark arrestor. Tank valves were equipped with flat plugs. The tank battery was diked and fenced with a locked man door. The proper, legible signage was located at the well and tank battery locations. The lease access was stable. No violations were observed.
