They notified me that natural gas was detected in the area of the four Metzenbaum houses and they were evacuated. I returned the call and was notified that Cutter was on site, they had shut the well down and the scene was safe. The well was shut in at the time of my inspection. I spoke to Cutter who said this well is on a 24 hour cycle. The well had unloaded 2 of fluid into the production tanks within one hour. This fluid is unloaded from the hole with natural gas. The large volume of fluid means a large volume of natural gas was needed to move the fluid. This gas was vented into atmosphere and accumulated in the area due to weather conditions. Also, per Cutter, the Dominion line pressure changed from 52 PSI to 48 PSI during the incident. Cutter is going to change the well cycle to three times a day. Inspection findings; The tanks are equipped with low pressure relief valves, a spark arrestor and a charcoal filter. A bull plug is out of the east tank. The production tank has a hard line plumbed from the side of the tank into the poly brine tank, as an over flow. 1501:9-9-05 (4) ( C) Vent stacks shall have a functional low-pressure relief valve. Line is in non-compliance with ORC 1501. Line needs removed from the tank or a relief valve put in place. Per the fire report, a large amount of vapors were rising from the vents of the 2 large tanks and also from a smaller tank located in fenced area. Lower Explosive Limit readings at the gated area was 65% and 100% near the tank vents. Remedial action needed: 1. Immediately remove vent pipe or install safety equipment. 2. Inspect hatch seals and replace as needed. 3. Insure that hatches are secured at all times when the well owner or his representative are not on-site. |

The signage at the tank battery needs to be replaced. The lease access was stable. I contacted Mark Turpak from Cutter about replacing the signage at the tanks. |
