Tank battery has lock on it, and is secured. Well head does not have lock on the man door. Lock on man door to the well head needs installed.
1017001300 2013-05-28 UrbanProduction Wells No Violations My inspection found the well to be equipped with a plunger lift unit. The annulus valve is open to a pop off valve that is plumbed into the production tank. The tank battery and well are in good repair and surrounded by a chain link fence. The tank battery consists of two 100 BBL production tanks, a 50 BBL poly brine tank, and a mechanical separator. The vent stack on the production tank is plumbed into a 55 gallon charcoal filter. The vent stack is then plumbed up and topped with a spark arrestor. The proper identification is located on the tank battery. The lease road is in stable condition.
1240326650 2009-04-20 Drill / Deepen / Reopen No Violations Pits have been closed. Drainage controls need repaired. Drainage controls at culvert have been blown out. Silt fence has been knocked down in a few locations. Area south west of well head needs mulched to prevent off site erosion. Called Warren and Dan Passerini requested that drainage controls be repaired and improved.
1314284944 2011-08-17 Production Wells Violations Noted I received a call from the Chester Fire Department the evening of 08/16/2011 about gas venting from a tank battery. They notified me that natural gas was detected in the area of the four Metzenbaum houses and they were evacuated. I returned the call and was notified that Cutter was on site, they had shut the well down and the scene was safe. The well was shut in at the time of my inspection. I spoke to Cutter who said this well is on a 24 hour cycle. The well had unloaded 2 of fluid into the production tanks within one hour. This fluid is unloaded from the hole with natural gas. The large volume of fluid means a large volume of natural gas was needed to move the fluid. This gas was vented into atmosphere and accumulated in the area due to weather conditions. Also, per Cutter, the Dominion line pressure changed from 52 PSI to 48 PSI during the incident. Cutter is going to change the well cycle to three times a day. Inspection findings; The tanks are equipped with low pressure relief valves, a spark arrestor and a charcoal filter. A bull plug is out of the east tank. The production tank has a hard line plumbed from the side of the tank into the poly brine tank, as an over flow. 1501:9-9-05 (4) ( C) Vent stacks shall have a functional low-pressure relief valve. Line is in non-compliance with ORC 1501. Line needs removed from the tank or a relief valve put in place. Per the fire report, a large amount of vapors were rising from the vents of the 2 large tanks and also from a smaller tank located in fenced area. Lower Explosive Limit readings at the gated area was 65% and 100% near the tank vents. Remedial action needed: 1. Immediately remove vent pipe or install safety equipment. 2. Inspect hatch seals and replace as needed. 3. Insure that hatches are secured at all times when the well owner or his representative are not on-site.
1348940635 2008-12-29 Drill / Deepen / Reopen No Violations Drill depth 3499, operator should be TD by end of day. Pits are in good repair. Lease road is stable. Drainage controls are in place. Permit ID sign is up.
1378310669 2017-05-30 UrbanProduction Wells No Violations I conducted a reinspection on this well pertaining to the tank battery location needing signage installed. During this inspection, the signage has been installed at the tank battery location and no violations were observed.
1627987999 2009-05-06 Production Wells No Violations Drainage controls have been repaired. Operator has began to ruff grade the area. Well head and tank battery has been fenced. Lease road is stable.
1921982086 2009-10-19 Production Wells No Violations Mike Cutter on site, working on well. Dominions sales line management is causing issues with well. Separator was iced up, operator was working on thawing it out. Mike stated that line pressures have caused pop offs to release. Locks are on man doors. Site is in good repair.
2008365864 2011-03-17 Production Wells No Violations 2 100Bbl tanks in the battery. Tanks were over 80% full. A retention pond has been constructed adjacent to the well. Pond is for the school. Identification sign has come off of the well fence. Dike was in good repair. Site was in good repair.
2029406872 2017-01-24 UrbanProduction Wells No Violations At the time of this inspection, the well was equipped with a plunger lift assembly and the valves on the wellhead were open. The well was fenced with a locked man door. The tank battery consisted of two 100 BBL production tanks, one 50 BBL poly brine tank and a mechanical separator. The valves on the separator were open and the chart showed production. The last reported production for this well was submitted in 2015. The production tanks had the correct lids and hatches. The vent stack on the production tank was plumbed into a 55 gallon charcoal filter, then plumbed up and topped with a spark arrestor. The tank battery was diked and fenced with a locked man door. The signage at the tank battery needs to be replaced. The lease access was stable. I contacted Mark Turpak from Cutter about replacing the signage at the tanks.
