0878489092 2018-02-12 UrbanProduction Wells No Violations I conducted an inspection on this well. The well is equipped with a plunger lift assembly and the valves on the wellhead were open. The well was fenced and the man door was locked. Last production records were submitted in 2015. The tank battery consisted of one 100 BBL production tank, one 50 BBL poly brine tank, and a mechnical separator. The valves on the separator were open and the chart mounted on the separator showed production. The production tank had the proper lids and hatches and the tank valves were plugged. The tank battery was diked and fenced with a locked man door. The fence has been repaired and vegetative maintenance has been conducted at the tank battery location. The proper, legible signage was located at the tank battery. The lease access was stable. No violations observed.
1337084424 2012-05-14 UrbanPreliminary Restoration No Violations My inspection today found the following: The lease road was stable and constructed out of durable material. The first 200 yards of the lease road share use with CEIs right-of-way. The operator has installed a cable gate at the entrance of the lease road, off of Camelot drive. The well is a plunger lift system well. The wellhead was in good repair. Wellhead is fenced, the man door to the wellhead did not have a working lock on it. The tank battery is fenced, there was a lock on the man door, that was not closed. Lock was closed after inspection. Tank battery has a 100 barrel production tank, a 1500 gallon brine tank and a mechincal separator in it. HB-278 equipment was installed on the tank battery. There is a drain pipe in the side of the dike with a shut off valve. Valve was open, I closed the valve during the inspection. Dike is in good repair and sufficient to contain a fluid release, when the drain valve is closed. Vegetation around the affected area is established. Evergreens the operator planted for landscaping and screening are established and have new spring growth on them. Identification sign is posted. Operator will be contacted by e-mail in regards to: locks and keeping the drain valve closed when site is not attended.
1440524593 2015-08-24 UrbanProduction Wells No Violations My inspection found a tree branch has fallen on the chain link fence damaging one section. The tank battery is over grown with vegetation. No other changes to the location since my last inspection. I contacted Mike Cutter the owner of Cutter Exploration about repairing the fence and removing all of the vegetation inside the tank battery. He stated he will have his well tender fix the issues by 8/28/15.

The last inspection report dated 8/24/2015 addressed the issue of the tank battery fence needing to be repaired due to a downed tree, and vegetation removal. The fence has been repaired and vegetative maintenance has been conducted at the tank battery location. The proper, legible signage was located at the tank battery. The lease access was stable.
1808397853 2011-03-17 Production Wells No Violations 100Bbl production tank and a 1500 gallon poly tank in the battery. Oil needs cleaned up behind the tank by the charcoal filter. Side of tank has oil leaked down the side of it. Well is weeping on the 2 inch line. Line needs cleaned and fittings tightened. Lease road gate needs repaired. Site has been graded, seeded and vegetation is established. Lease road is stable. E-mailed operator in regards to the well.

The annulus valve is open to a pop off valve that is plumbed into the production tank. The tank battery and well are in good repair and surrounded by a chain link fence. The tank battery consists of a 100 BBL production tank, a 50 BBL poly brine tank, and a mechanical separator. The vent stack on the production tank is plumbed into a 55 gallon charcoal filter. The vent stack is then plumbed up and topped with a spark arrestor. The proper identification is located on the tank battery. The lease road is in stable condition.
