0338953589 2007-03-12 Preliminary Restoration No Violations Well head has been installed. Fence is up around well head. Drainage is contained on site. Well is a rabbit well. Pits have been filled in. Site has not been graded, site is snow covered. Permit sign up at the entrance of the lease road. Lease road is stable. Tank battery has two wells. Two 110 BBL tanks and one 50 BBL tank on site. Two separators on site one mechanical other may be heated. The Farmer tank overflowed. Called company had problems with pumper, cleaned up site 03/13/07. Spoke to company regarding heated separator and spacing. They will be checking on it.
0807463894 2007-03-14 Production Wells No Violations Operator on site cleaning up oil spill at tank battery. Oil from the side of the tanks has been cleaned off. Operator was spreading peat to soak up oil on the ground. Tank battery didnt over flow. Operator stated tank had 9 feet of oil in, gas pressure is such that it caused oil to spray from tank. Operator is going to take steps to modify the tank to prevent the problem in the future. Will need to meet with operator regarding separator. Separator appears to be a heated separator within 50 of the tank battery.

Inside the board on board fencing there were small trees that extended above the fencing. The vegetation at the well site needs to be maintained consistent with reasonably prudent operations.

1371041434 2013-06-12 UrbanProduction Wells No Violations My inspection found the well to be equipped with a plunger lift unit. The annulus valve is open. The wellhead is surrounded by a wooden fence. Two landscape mounds in place with three trees. The landowner stated that three of the pine trees had died and have not been replaced. The tank battery consists of two 100 BBL production tanks, a 50 BBL brine tank, and two mechanical separators. The production tanks vent is plumbed into a 55 gallon charcoal filter then up into a spark arrestor. The well head needs an identification sign on it and the trees replaced. The proper identification is located on the tank battery. The lease road is in stable condition.

1898651466 2017-04-04 UrbanProduction Wells No Violations At the time of this inspection, the well was equipped with a plunger lift assembly and the valves on the wellhead were open but the valve on the 2 inch flowline was closed. There was a domestic line off of the 4.5 inch casing with a meter, drip bottle and regulator. The valve on the 2 inch line was open. The last reported production for this well was up to date. The well was fenced with a locked man door. The tank battery consisted of two 100 BBL production tanks, one 50 BBL steel brine tank and two mechanical separators. Valves on the separators were open. The production tank vent stacks were plumbed into a 55 gallon charcoal filter and topped with spark arrestors. The production tanks had proper lids and hatches and the tank valves were equipped with flat plugs. The tank battery was diked and fenced with a locked man door and is shared with well 2-1741. Proper, legible signage was located at the tank battery but needs to be installed at the well. The lease access was stable. I contacted Mark Turpak from Cutter Exploration about installing a sign at the well location.
1963416230 2007-06-18 Preliminary Restoration No Violations Site has been ruff graded. Topsoil is stockpiled around the well head. Spoke to Lynn, Cutters consultant Mr. Farmer wants to sue Cutter over the time it has taken for restoration. Well was spudded 10/11/2006 restoration must be completed by July 2007. Lynn stated that the site has been too wet to work. Site is very dry at this time, the clay has started to crack. There is a damp spot were the dog house of the rig was. Lynn stated that there was a spring at that location. Operator still has silt fence up. Well head is fenced and painted. Well head needs to be signed.
