0111459474 2016-03-16 UrbanProduction Wells No Violations My inspection found the well to be equipped with a plunger lift unit. The annulus valve is closed. The tank battery, separator, and wellhead are surrounded by a wooden fence. The tank battery consists of a 210 BBL production tank and a 100 BBL brine tank. The dike around the tank battery needs rebuilt. The load lines on the tanks and the man doors on the fences need locks installed. An identification sign needs posted on the tank battery fence. The heat treated separator has the proper spacing from the wellhead and the tank battery. The lease road is in stable condition. I have contacted the company about correcting the issues.
1185465989 2007-07-26 Preliminary Restoration No Violations Well head has been installed. Tank battery is under construction, one 210 and one 100BBL tank on site. Lease road is stable. Pits have been closed. Silt fence is up. Permit sign is posted on the tank battery.

Called Tim Reily from John D, well doesnt have a charcoal filter or spark arrestor on it. Requested that Tim finds out why. Area where flow line was installed, has settled, needs filled in and repair seeded. Man doors were locked. ID sign is up.
1241699299 2009-05-06 Production Wells No Violations Tank battery is not in compliance of HB-278 on site with Jay Cheslock. 210 BBL tank is above the fence more than 3 feet.
1377118479 2007-06-01 Drill / Deepen / Reopen No Violations Drill depth 3261. Pits are in good condition. Safety fence is up. Drainage is contained on site.
1481729396 2016-11-15 UrbanProduction Wells No Violations I conducted an inspection on this well. The well is equipped with a plunger lift assembly and the valves on the wellhead were open. The last reported production for this well was in 2015. The well was fenced. The tank battery consisted of one 210 BBL production tank, one 100 BBL brine tank and a heater treater separator. Proper spacing was observed between the heater treater separator and production tank. The tanks were equipped with the proper lids and hatches and the tank valves were plugged. There was a charcoal filter plumbed into the 210 BBL and spark arrestor was plumbed into the vent stack. The tank battery was diked and had the proper, legible signage. The lease access was stable.
2044962789 2013-06-07 UrbanProduction Wells No Violations My inspection found the well to be equipped with a plunger lift unit. The tank battery and well are in good repair and surrounded by a chain link fence. The heat treated separator has the proper spacing and is surrounded by a wooden fence. The tank battery consists of a 210 BBL production tank and a 100 BBL brine tank. The 210 BBL tank is more than 3 feet above the fence. The vent stack on the production tank is plumbed into a 55 gallon charcoal filter and then plumbed up and capped with a spark arrestor. The proper identification is located on the tank battery. The lease road is in stable condition.
