0242182260 | 2016-11-29 | UrbanProduction Wells | No Violations | I conducted an inspection on this well. The well is equipped with a plunger lift assembly and the valves on the wellhead were open. The last reported production was submitted in 2010 for this well. The tank battery consisted of three 100 BBL production tanks, one 50 BBL poly brine tank, and two mechanical separators. The tank battery is shared with well 2-1660. The sales line within the tank battery had two charts dated 11/7/2016 and showed production. Valves on the mechanical separators were open and the tank valves were plugged. The tank battery was diked and fenced but needed a lock on the man door. The proper, legible signage was located at the well and tank battery locations. The lease access was stable. I contacted Jim Pavlich 440-693-4419 of Grimsby Exploration about adding a lock and updating the production records. |
0321391696 | 2018-01-10 | UrbanProduction Wells | No Violations | I conducted an order follow- up inspection on this well to see if the well was in production. The well is equipped with a plunger lift assembly and the annulus and flow line valves were open. Production records for this well were current. The tank battery consisted of three 100 BBL production tanks, one 50 BBL poly brine tank, and two mechanical separators. The tank battery is shared with well 2-1660. Valves on the mechanical separators were open. The tank battery was diked and fenced with a locked man door. The proper, legible signage was located at the well and tank battery locations. The lease access was stable. I contacted Swallen, Lawhun and Co. who is the firm that does the accounting on behalf of Grimsby Exploration, Inc. and spoke with Kristie Lear, regarding liability insurance. Ms. Lear forwarded a copy of the liability insurance to me, which I forwarded to DOGRM, Surety and Bonding section. |
1339962171 | 2013-05-29 | UrbanProduction Wells | No Violations | My inspection found the well to be equipped with a plunger lift unit. The well is in good repair and surrounded by a chain link fence. The tank battery consists of three 100 BBL production tanks, a 35 BBL poly brine tank, and two mechanical separators. The fence needs repaired around the tank battery. I spoke with Jim Pavlic the well tender for Grimsby Exploration and he stated that he would have the fence repaired. The lease road is in stable condition. The proper identification is located on the tank battery. The lease road and tank battery are being shared with permit number 2-1660. |
