0518587004 2018-02-08 UrbanProduction Wells No Violations I inspected this well as part of routine inspection activity. At the time of this inspection, fittings and connections at the battery were secure and free from leakage or problems. The pump jack system had been removed as part of work-over but the well remained in production. The battery enclosure was locked and secure. The enclosure at the wellhead had been dismantled for work-over. Production reports were current. Proper bonding and insurance was in place. The surrounding areas were covered in several inches of fresh snow cover.

1450297545 2015-12-16 UrbanProduction Wells No Violations I inspected this producing well as part of routine inspection activity. The well and associated battery are located in a densely wooded area on the south side of SR 422 with residential homes beyond. At the time of this inspection, the wellhead and tank battery enclosures were locked and secure. ID signage was present at the head of the lease road, and at both the wellhead and battery locations. Secondary containment was present and intact at both the wellhead and battery. Load-lines were plugged, locked and secure. Fittings and connections were secure with the following exceptions: I observed a minor leak of gas from the top fitting on the domestic meter located in the battery enclosure, and a missing closure wing nut at the production tank hatch lid. I contacted CJ Cutter regarding the gas leakage and missing wing nut. He advised that the items would be addressed within 48 hours.
1477945572 2016-10-31 UrbanProduction Wells No Violations I inspected this producing well as part of routine inspection activity. At the time of this inspection, I observed the following: a pump jack unit in a board-on-board enclosure and associated tank battery in a separate board-on-board enclosure. The enclosure gates were locked and secure. Load-line valves were plugged and secure with locks. The electrical panel was locked and secure. Tank lids were closed and secure. The secondary containment was intact and dry with no signs of leakage, spills, or other problems. The areas surrounding the production equipment consisted of trees and wooded areas. I walked the surrounding areas and found no signs of spills, leaks, or other evidence of environmental degradation. During a previous inspection, I had noted that the wing nut was missing from the production tank lid and there was a small gas leak at the domestic meter. These items had been remedied at the time of this inspection.
