0293017759 2018-10-09 Final Restoration No Violations I was on location to evaluate the location for Final Restoration following plugging and abandonment that was completed on 10/1/18. At the time of this inspection, I noted the following: the well plug was stable and showing no signs of leakage or problems at the surface. The tank battery had been dismantled and removed, and the area was re-graded. The areas around the well were to be left ungraded and re-vegetation was being waived per agreement with the landowner. In addition, the access road was to be left in place, and the flow-lines and sales lines left in place per waiver agreement, following evacuation of residual gas and fluids. Final Restoration is passed.
0328695260 2014-03-31 UrbanProduction Wells No Violations The producing unit is comprised of a wellhead with pump jack in a locked chain-link enclosure. The well is producing and I observed no signs of leakage or problems. The aboveground storage tank battery is located near the gated lease road entrance and is in a locked chain-link enclosure. The battery is comprised of 1 100-bareel steel production tank, 1 50-barrel steel brine tank, and separator unit. There is a Dominion metering run outside the battery which appears to be functioning normally. Load-outs are bull plugged and locked, there is secondary containment. I did not observe signs of leaks, spills, or other problems.

1362063131 2013-02-25 Preliminary Restoration No Violations The producing unit consists of the following elements: A production wellhead and tank battery. The production wellhead consists of a pumpjack system in a 6 chainlink enclosure. There was 80 pounds per square inch (PSI) of pressure at the 4 1/2 casing at the time of my inspection. The tank battery is located 100 feet east of the wellhead, and is comprised of 1 100-barrel (BBL) tank, 1 50-BBL tank, mechanical separator, and associated piping and appurtenances enclosed in an 8 enclosure. The unit is lacking the HB278 production sfaety equipment. I contacted Bakerwell and advised them of the need for the equipment.

At the time of this inspection, fittings and connections at the well and battery were secure and free from leakage or problems. The load-line valves were secure and locked. Tank lids were closed and secure. Secondary containment as intact and free from signs of release, leaks, or problems. The well was in active production. Production reports were current. Bonding and insurance were active and current. I walked and inspected the areas surrounding the wellhead and battery and found no evidence of impacts from leaks, releases, or degradation to the natural greenscape.

2099566244 2015-09-22 UrbanProduction Wells No Violations I inspected this producing unit as part of routine inspection activity. The well and battery are located on an otherwise undeveloped parcel. The area immediately surrounding the well is comprised of woods and meadow grass. Access to the well and battery are afforded by an unpaved, dedicated lease road which enters from County Line Road and travels 300 feet to the unit. At the time of this inspection, the well is in production and records are current. Fittings at the wellhead and battery are secure and free from signs of leakage. Secondary containment is intact and free from staining or other problems. ID signage is present and accurate. The access road is in good condition and well drained. Areas surrounding the wellhead and battery are free from signs of impact relating to production activity.
