On 07/26/16 at approximately 1330 hours I conducted an incident response inspection at the EQT Hildebrand well pad in response to a BMP blowout at the site. The Act 9 sign was in place and all required documents were present. The site was being restored at the time of the BMP failure. A large rainfall event had occurred, overwhelming the sediment trap and causing the downslope wall of the sediment trap to fail. The resulting water, rock, and sediment broke through multiple silt socks leaving the LOD and entering a stream. A large amount of rock and gravel was found in the stream bed approximately 150’ from the sediment trap failure (see photographs). A pond approximately 200-250’ downstream was discolored from the sediment released from the BMP failure. At the time of inspection an excavator was repairing the breach in the sediment trap wall. On 10/14/16 at approximately 1100 hours I conducted a compliance evaluation inspection at the EQT Hildebrand. The Act 9 sign was in place and all required documents were present at the site. In July 2016 the site had a sediment trap blowout, resulting in rock and sediment breaking through multiple silt socks and impacting a nearby stream. An emergency 105 permit was obtained by EQT and the material was removed from the stream. The sediment trap and erosion had been repaired at the time of inspection and the site was exhibiting new vegetation. There were no violations observed at the time of inspection. |

On 03/06/16 a complaint was received by the Department alleging BMP failures at the Hildebrand well site. On 03/07/17 at approximately 1330 hours I, along with Water Quality Specialist Supervisor John Carson, conducted a complaint inspection of the site. The Act 9 sign was in place and all required documents were present. Gravel from the site was found across Bowser Road. This is a violation of 102.5(m)(4) for impacts outside of the limits of disturbance. A black corrugated pipe that was not shown on the E&S plans was observed on the right side of the access road immediately upon entry to the site. It is requested that EQT provide information on this pipe in the NOV response. The rain garden was not functioning as designed and is at risk of failure. The leak around the riser structure in the rain garden had been repaired. The rain garden appears to contain sediment from poorly stabilized adjacent slopes. The slopes appeared to be failing, depositing sediment into the rain garden and causing a loss of volume. Additionally, erosion was occurring along the southwestern rip rap channel into the rain garden. It is recommended that this be repaired to prevent further erosion and volume loss. It is requested that EQT contact Donna Davis to discuss maintenance of the BMP. It is requested that EQT contact Donna Davis regarding the cross drain causing erosion into the pond on UNT to Castile Run. The silt socks that were previously undercut or overtopped had been maintained to prevent undercutting, but sediment was still close to or at the top of the silt soc |

Poorly vegetated soil and accelerated erosion were observed where silt sock was removed in the area of the restored sediment trap. Multiple small erosion rills were observed in the vegetated area upslope. It appeared the rills existed and were not repaired prior to the vegetation grew and covered the rills.
