0602799714 2009-07-29 Orphan No Violations Rods and tubing being removed from well. The well has wooden rods. Rods broke at about 500 foot depth. Tubing in the well is deteriorated and most joints had to be cut when removed. Tubing broke at a depth of approximately 1000 feet. Discussed situation with ODNR supervisor Jeff Fry. Decision made to try to fish rods and tubing from well. Operator ran tool down well to try to catch rods. If this is not successful will try to run 2 inch tubing down well, past rods to 1000 foot depth and cement from that depth.
1119538552 2009-02-18 Orphan No Violations Checked well spill location after steady 6 hour rainfall. No new discharge from well. Absorbent oil booms on site have been flipped so clean side down. The water elevation in the farm field was up with a slight trickle discharge to the culvert under Poe Road. An absorbent oil boom has been staked at the inlet of the culvert. No signs of oil in the road ditch.
1234467414 2009-02-12 Orphan No Violations Responded to oil spill which occurred in farm field owned by Owen McKee, next to home at 15240 Poe Road. Heavy rainfall on 2/11/09 apparently caused fresh water to break into an orphan well causing oil to overflow into a farm field. Due to flooding from the rainfall, oil spread across the field into a road drainage ditch. There was no oil noted discharging from the well at time of inspection, however there was oil dripping from a pipe associated with the well. Rods and tubing were in well. There was also an open casing approximately 10 feet from the well. USEPA and OEPA responded to the spill. Booms were placed around the well and in the field to contain the oil. Crew was on site cleaning up oil. Wood county Health Department was also on site at time of inspection. There are 2 houses near the well site. One landowner reported that oil was noted in well water several years ago. Two other wells were noted near the leaking well and information from adjacent landowner Tony Christoff indicated there were supposedly 9 wells in the field.
1234893420 2009-02-13 Orphan No Violations Met on site with OEPA inspector Mike Gerber, USEPA inspector Jim Augustyn and Wood County Health Department supervisor Jerry Bingham. Clean up of oil continuing. Discussed water samples Wood County Health Department will take from water well next to site. Mike Coyer of Black Swamp Oil Field Services stopped at site. Mike inspected well and it was decided to hire Mike to remove all pipes and equipment from well so a PVC casing could be placed on the well. The open casing near the well was an anchor for the pump jack. There were two, 2 inch openings on the well head where Mike thought oil had discharged. Oil was noted to the top of the lower opening in the well casing, indicating more oil in the well. Oil dripping from a 2 inch pipe at the well apparently came from the discharge from the leaking well, not another well in the field.

1238609773 2009-04-01 Orphan No Violations Oil booms have been removed from site. No standing water in farm field but areas are still wet and saturated. There is some residual oil on the field where oil coated corn field stubble at the edge of where water was impounded at one time. Oil did not saturate into ground. No signs of leakage on well. Bids are being requested to plug 2 wells in the field.
1249304981 2009-07-30 Orphan No Violations Operator made several attempts to fish out rods and tubing remaining in well, but was unsuccessful. Two inch tubing was run into well and reached depth of 980 feet where mud was encountered This is also where old tubing broke off in well. Well will be plugged at this depth since milling out old tubing and mud would be expensive and would add little benefit to project.
1249482556 2009-08-04 Final Restoration No Violations Well plugging complete. Operator cut off well 3 feet below ground elevation and attached ID plate to top of well. Pit was then backfilled. Site has been graded and all equipment removed. Crop area disturbed from well plugging activity is approximately 0.15 acres.
1272292934 2009-08-03 Plug / Plug Back No Violations Operator pumped 3 yards of Class A cement through 940 feet of 2 inch tubing into well on 7/31/09. Water circulated to pit. Cement job witnessed by ODNR inspector Sam Faust. Cement set up at 920 feet which indicates most of the cement went into the formation. Rather than pump Class A cement for the next plug, 9 sack grout will be used. Previous well pluggged on site (34-173-61226-0000) lost much of the 13 yards of Class A cement to formation during plugging. Operator gravity fed 7.5 yards of 9 sack grout into well and displaced fluid to plastic lined pit. Well lost circulation after 3-4 yards of grout were gravity fed into well. Two inch tubing was removed from well. Bottom joint of pipe was plugged with sand and rock. Obstructions removed and tubing run back into well where top of cement was located at approximately 250 feet. Continued reverse grouting until cement rose to surface of well.
1405416195 2009-02-17 Orphan No Violations Water in farm field has dropped. Some of oil boom is now on dry ground. Culvert from farm field to drainage ditch on Poe Road is exposed. No oil was noted in the ditch. There is no oil boom or protection at the culvert however there may be residual oil on the farm field. Check of well site indicated no further discharge of oil on the ground surface. Spoke to Jim Augustyn of USEPA by phone. Oil booms may be removed in a week or so if no further oil discharge is noted from well. As noted last week, there is still oil in the well and further discharge may be possible if well is not plugged. Contamination of adjacent water wells is possible through both the lower limestone formation and upper sand in farm field. Discussed placing well on I&O emergency program with Mike McCormac and Jeff Fry.
