1141070589 2006-02-23 Production Wells No Violations The well is located on property whose ownership is currently being decided in probate court. The 2 parties involved in the case are Donna Carlton and James Atkins whose aluminun foundry, SMC, has used gas from the well in the past. Gas from the well also apparently was used to heat the Carlton home. This well is not permitted or bonded with the State of Ohio. According to both parties noted above, gas from the well is not currently being used. Enforcement action will be taken after further discussion with Columbus office.

1687383426 2006-05-16 Production Wells No Violations Well depth was checked. Rig set up on well could not pass bailer beyond 350-400 feet due to an obstruction. Hand line was dropped down well and determined that depth was approximately 1500 feet. There appeared to be approximately 500 feet of fluid in the bottom of the well. Operator will use 2 inch tubing to pump cement into the well tomorrow.
1933397212 2006-06-06 Orphan No Violations Well was plugged and witnessed by inspector Tom Benko. The well is located inside a fenced area near the foundry building. Noted ID plate on ground surface in cement slab. Fence removed where storage tank was removed has not yet been repaired. Dave Barndt said pieces to repair fence were missing from site. New parts will be obtained and fence repaired.
1948714868 2006-05-03 Production Wells No Violations Spoke to Mrs. Atkins about State of Ohio Orphan Well Program Plugging Application and Right of Entry Release submitted for plugging the 3 wells on the properties. Revised form. Checked well site with an employee to identify tanks near well that are currently being used by the foundry operation. There is a natural gas tank and fuel oil tank currently being used. A seperator tank, associated tank and 2 pump jacks near the well were part of the oil and gas operation. There is also a large oil storage tank on the property that was part of the oil and gas operation.
