0619822739 2008-11-19 Orphan No Violations Inspection with Jeff Fry. Reviewed down hole video shot by operator. While trying to run smaller bit into hole as discussed in previous inspection, the bit moved into the annular space between the 6 inch casing and borehole. This caused the casing to bend over into the inside of the casing at approximately 55 feet depth. The video also revealed that a portion of the 6 inch casing was missing from depth of 28 feet to 35 feet. The remaining casing in the hole was deteriorated. Operator can now not get past the 55 foot depth with 4.5 inch bit. There appears to be sufficient area to run 1.5 inch tubing past the obstruction at 55 feet. It was decided to plug the well as is rather than try to mill out obstruction.

1250520616 2008-11-24 Plug / Plug Back No Violations Operator ran 1.5 inch tubing to depth of 75 feet and pumped 25 sacks of cement into well on 11/21/08. Plug was witnessed by inspector Sam Faust. Cement set up at 62 feet. An additional 22 sacks of cement were mixed and poured into well on this day. Cement set at top of casing. Casing was cut off at 4 feet below ground and ID plate attached. Pit backfiled while on site.
1997760998 2008-11-13 Orphan No Violations Inspection with Mike McCormac and Jeff Fry. Spoke to Bob Harpster and Dave Barndt on site. While trying to clean out the well, an obstruction was encountered at depth of approximately 200 feet. Operator thought that obstruction was from side wall. A smaller bit wil be tried to pass through the obstruction. The plugging permit is for permit 34-173-61129-0000. The operator is drilling out the wrong well. Both of these wells are for landowner Rick Coleman but are on different properties. Will allow operator to continue with plugging well 34-173-61135-0000 with existing contract for well 34-173-61129-0000.
