Record Last Modified 2010-10-20 Inspected by Cynthia van Dyke 10/31/2008. Inspection due to oil spill. Well is being pumped by jack, small (40 bbl) tank nearby sitting of edge of run. Reportedly well drilled to Cow Run, has been produced by landowner for very long time. Received call 10/31/08 - believe spill could have been as early as 10/26 or 10/27 before rain - owner reports tank was almost full and now has about 1' in it. Tanks has dike and no oil - maybe cut line? Outside dike area but estimated 10 -

0732673958 2009-01-16 Production Wells No Violations Inspection this day found dike still in creek from oil spill - he told me he got it out. Weather has been so bad cant get a lot done. Tank still needs IDs, has fair to good dike, well with jack needs serviced - cant produce.
1143307188 2010-04-23 Production Wells No Violations Inspection this day found well with jack shut-in - waiting on service rig. Little oily around well, tank has no IDs, poor dike.
1225715884 2008-10-31 Production Wells No Violations Inspection due to oil spill - went down into run about 500. Well is being pumped by jack, small, 40? Bbl tank nearby, sitting on edge of run. Cant write NOV for dike until get permit no. Reportedly, old well drilled to Cow Run, has been produced by landowner for very long time. Received call on 10/31/08 - believe spill could have happened as early as 10/26 or 10/27 before rain - owner reports tank almost full and now has about 1 in it. Not sure what happened as tank has a small dike and it has no oil - maybe line cut? Outside dike area but I estimate 10-15 bbl. Lost. Run dry in almost all except for a few pools and these are holding oil. UniGas showed up in AM, cover run with pads and soaked up a lot of oil, boom put below spill, then dug hole with backhoe and washed run with 2 loads of fresh water very late in evening. Were going to go back out on 11/1 and finish clean-up.
1231444142 2009-01-05 Production Wells No Violations Oil spill clean-up finally done. All pads picked up, stream has only a sheen on it, dike at bottom has been removed so that stream now flowing again. Operator sending in papers to get well put in her name.
2029107573 2008-12-10 Production Wells No Violations Review progress of oil spill from last month - waiting on a good rain and now have it. Crew needs to pick up pads, put clean down to sop up remaining oil now coming out of the rocks and flowing downstream. Talked to Daryl Bowersock, who was in charge of clean-up and he said will finish it up on Friday.
