0355264944 2016-01-05 Production Wells No Violations Inspected this location due to seeing fluid in the tank dike that was not frozen with an air temperature of 27 degrees. Upon field testing the fluid, it was observed that it is produced brine that is being discharged into a pit in the earthen dike from a small knockout tank. The amount of fluid present in the pit is estimated to be less than 1 bbl. I spoke with the landowner, Mr. Mike Dye, who claims he owns the well and uses the gas for his farm. I told Mr. Dye that disposing of produced brine in this manner is illegal and needs to stop immediately. Mr. Dye agreed and stated that he would have his son re-plumb the knockout tank into the 50 bbl production tank and have the brine sucked out of the pit for disposal. I also told Mr. Dye to mix in lime with the soil in the dike to encourage bioremediation of residual chlorides. Mr. Dye agreed to comply and stated he would call me when cleanup is complete.
0386466459 2016-05-11 Administrative Inspection No Violations Spoke with landowner Mike Dye in regard to this well. Mr. Dye stated that his son Bruce has been working toward attaining exempt domestic ownership of the well. A request has come through from Central Office to perform an inspection for exempt domestic pre-transfer. I will contact Bruce Dye and set up a time to inspect the well.
1081758213 2016-06-07 Exempt Well Pre Transfer No Violations On location with DOGRM ER Coordinator Nathan Lane to perform an exempt domestic pre-transfer inspection. Upon inspection, it was observed that the well is currently in production and is in use by the landowner for domestic gas purposes. The well consists of a pumping unit standing over 6.625 production casing and 2.375 production tubing. The tank battery consists of a 50 bbl production tank with a small knockout tank. All produced fluids are being pumped into the production tank. There is a small pit below the tank which is no longer in use. There is a containment dike around the tank. I have spoken with the landowners son Bruce Dye and instructed him to post proper ID information at the well and tank. No significant violations noted. The well may be transferred for exempt domestic ownership.
1105609645 2018-10-18 Production Wells Violations Noted On this day, I found the dike area containing less than 5 barrels of oily production fluid. At some point in time, the fluid appears to have breached the dike and entered a ditch causing pollution and contamination. The amount outside the dike is unknown due to the time frame at which the incident may have occurred. No pooling of oily fluid is witnessed throughout the drainage ditch. The production facility consists of a 50 barrel steel tank and knockout tank, as referenced in previous inspections, contained within an earthen dike. The well is registered as a domestic well to Michael and Sara Dye. I spoke with Mike Dye during the inspection, and he indicated that there was road work being conducted near his production facility that had initiated the incident. He indicated that he and his son will be working to pump the fluid from the dike, re-gravel the area, and reconstruct the dike to insure a future incident doesnt occur. It was also requested to scrape and dispose of any contaminated soil outside of the dike. Mr. Dye agreed to start work immediately, and contact me when the work was completed. A follow-up inspection will be conducted. Additionally, identification will need to be posted at the wellhead and tank indicating the owner, well name, permit number, and emergency contact number.
1542114931 2018-11-13 Production Wells No Violations On this day, I am conducting a compliance notice follow-up inspection. Progress is being made to gain compliance of the well. There is (1) 550 gallon polymer tank now present, that Mr. Dye will be utilizing for brine storage within the dike area. I spoke with Mr. Michael Dye during the inspection, and he indicated that he has hired a contractor to conduct the work to scrape the contaminated soils, reroute the drainage lines, rebuild the dike, and post identification. He also stated that the knockout drain lines, are no longer in use, and all fluid is running into the 50 barrel storage tank.
1544113596 2018-12-06 Production Wells No Violations On this day, I am conducting a compliance notice follow-up inspection. No progress has been made since the previous inspection. The 550 gallon polymer tank has yet to be connected, and rain water has since collected within the dike area with a slight sheen on the surface. I spoke with Mr. Dye during the inspection, and he indicated that he will have a contractor onsite 12/7/2018 to commence the work, in order to gain compliance. Additionally, he stated that the water will be removed from the dike on the same day.

The pollution and contamination issue at the tank has been resolved, the flowlines have been removed from the drainage ditch, the polymer tank is in place, and identification will be posted this week. The compliance notice will be resolved once the identification is in place.

1722913980 2016-01-06 Production Wells No Violations Met at this well location with landowners son, Mr. Bruce Dye, to discuss what needs done to make their tank battery legal. I explained to Mr. Dye that he has the option to eliminate the knockout tank and directly plumb all flow lines into the 50 bbl production tank, or keep the knockout tank and set a brine storage tank. I also explained to Mr. Dye that the soil in the dike must be treated with lime once the brine laying in the dike is pumped out and properly disposed of. Mr. Dye agreed to have the work done as soon as possible and will contact me when he is finished. I was accompanied at this meeting by DOGRM Hydrogeologist Jake Glascock.

I spoke with Mr. Dye during the inspection. The oily fluid has been removed from the dike, and gravel has been laid within the containment area. Mr. Dye indicated that he will keep moving forward to gain compliance. I reiterated that I would like any residually contaminated soil to be scraped and removed, the containment berm to be properly constructed, and identification be posted. Additionally, I requested to have the drain lines from the knockout redirected into a brine storage tank, and Mr. Dye agreed to have it done.
