0000335875 2018-10-02 Production Wells No Violations On this day, I am conducting a compliance notice follow-up inspection. The wellhead is properly identified and resolves the identification compliance notice. I found the well equipped with casing, tubing, production valves open, and a pumping unit with a gas engine. The unit was pumping during the inspection causing some pollution and contamination around the wellhead and down the hillside, near the well. The production facility consists of (1) 100 barrel steel tank. The facility is shared with permit number 6-6297. I spoke with Mr. Layton, during the inspection, and he indicated that he operates this well and receives domestic gas. I explained the issue that I found, and that it will need to be resolved immediately. A follow-up inspection will be conducted. The production records are current. Updated pictures were taken.

0997770241 2018-02-06 Production Wells Violations Noted The inspection this day revealed that this well is equipped with a pumping unit, 4 1/2 inch diameter production casing, and production tubing. The valves are open on the wellhead and the unit was pumping. The tank battery for this well is equipped with a 100 barrel steel tank. There is no visible identification posted on the wellhead or the tank. In accordance with 1501:9-9-05(A)(10) all wells must be legibly identified in a conspicuous place on or near the wellhead or storage tanks with the owner, lease name, well number, permit number, county, and emergency contact information. I spoke with Mr. Layton about this well and he informed me he gets some oil off the well but primarily uses it for house gas and, that his well and 66297 lead into the 100 barrel steel tank. I informed Mr. Layton he must post identification on the tank and wellhead of each of the wells on the property and he has 10 days to have them posted.
1402872919 2017-07-26 Production Wells No Violations The inspection this day revealed that this well is equipped with a pumping unit, a 4 1/2 inch diameter production casing, and 1 1/2 inch diameter production tubing. The valves on the wellhead are open at the time of inspection. The tank battery for this well is equipped with two (2) 100 barrel steel tanks with no identification on the tanks. There are no issues to report at this time and the well is in production at the time of inspection.
