0390379457 2018-10-23 Production Wells No Violations On this day, I am conducting a compliance notice follow-up inspection. I found the well equipped with casing and tubing, but no pumping unit attached, as indicated in previous inspection reports. The casing valve is closed and attached to a 1 inch diameter plastic flowline, and the tubing is venting to atmosphere. I spoke with Mr. Layton, during the inspection, and he indicated that he does not produce the well. There is thick vegetation surrounding the well. Layco reported zero (0) production from 1998-2014. There is no identification at the wellhead.
0406390633 2017-07-27 Production Wells No Violations The inspection this day revealed that this well is equipped with a pumping unit, 4 1/2 inch diameter production casing, and 1 1/2 inch diameter production tubing. The valves are closed at the time of inspection. I was unable to located any associated tanks with this well at the time of inspection. The well is idle at the time of inspection.
1702308029 2018-02-07 Production Wells Violations Noted The inspection this day revealed that this well is equipped with a scissor jack, 4 1/2 inch diameter production casing, and production tubing. No identification is legible on the well. The valves are closed on the wellhead and, Layco has reported zero (0) production from 1998 to 2014 on RBDMS production records. When I spoke with Mr. Layton yesterday he says he has never produced this well and he does not own it. I told him he should get into contact with Bill Grubaugh and talk to him about the wells he does not own and as to why they are on his bond. I also informed him the violations I found on the wells would still be issued to him as according to the State he is the one who owns the wells. There is no identification posted on the wellhead or the tank. In accordance with 1501:9-9-05(A)(10) all wells must be legibly identified in a conspicuous place on or near the wellhead or storage tanks with the owner, lease name, well number, permit number, county, and emergency contact information. This well is not in production and is idle. The well owner/operator shall submit a written plugging schedule to Brian Sefcik at 5880 Memory Road Zanesville, OH 43701 within 30 days, per ORC 1509:13(B)(7), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied.
