0508274075 2016-11-15 Preliminary Restoration No Violations The purpose of my inspection today is a status check on a preliminary restoration. During my inspection the well was producing and is equipped with a production tree wellhead. The production equipment consists of six 500 barrel production tanks, two glycol dehydration units, and a flare stack. Production equipment is shared with 2H 3416729761. The pipeline tie in located by the lease road has been completed and the location has been resorted. The well pad has a contamination free, gravel surface, with no erosion issues. All drilling equipment has been removed from location. Location has met 1509.072 A requirements.
0516430553 2015-09-28 Drill / Deepen / Reopen No Violations HAD Drilling Contractors completed drive pipe/conductor operations for this well over the weekend. HAD drilled 30 drive pipe into competent bedrock at 95-98 and proceeded to drive the pipe to 118 in depth. The concentric drilling rig is now standing over the cellar of P#29758 and will commence drive pipe operations for that well tomorrow.
0655910632 2016-05-10 Administrative Inspection No Violations Matt Wolf of PDC Energy called at 11:07 am to notify that Halliburton is moving onto the Mason Pad to begin stimulation operations. They will start with the 2-H and will perform a 27 stage frac before moving to the 4-H. Operations are scheduled to begin at 9:00 am on 5/11/16.
0940024944 2019-05-06 Production Wells No Violations I was on location today to do a field inspection on this well. At this time the well does appear to be producing out of the tubing wellhead and the casing wellhead. There are no violations at this time.
1089925332 2016-02-05 Drill / Deepen / Reopen No Violations On location to check status of drilling operations. Precision Drilling Rig #110 had damaged kelly hose and is currently being repaired. PDC Company Man Dave Gray stated they landed low with the bottom of their curve and were in the Lexington/Trenton Limestone. BHA will be tripped back in this evening to slide back into the pay zone and continue the lateral to TD. Current 8.75 hole depth 7911 with an azimuth of 3.67 degrees and inclination of 93.07 degrees.
1145528224 2015-09-24 Drill / Deepen / Reopen No Violations On location to witness drilling of 30 drive pipe/conductor for this well. Upon arrival, HAD Drilling Contractors had set 20 of drive pipe and were welding up another joint to take them to 40. HAD began drilling the 2nd joint into the ground and large rocks plugged off the blow line, causing back-pressure to force the rotating head off of the drive pipe. HAD cut the welded section of pipe off, laid it down, and began working on the blow line to clear it. HAD will switch from a tri-cone bit to a larger 28 hammer to avoid running into the same problem again. Drilling will continue tomorrow morning.
1352529910 2015-11-12 Administrative Inspection No Violations Received call from Mr. Larry Howell of PDC Energy at 7:11 am stating that the 12.25 section has been drilled to 4337 and drilling has ceased. The BHA will be laid down and Hall Rig #5 will skid to P#29760 to spud the 24 surface hole. The 12.25 open hole will be loaded with heavy brine and the well will be temporarily capped until a larger rig is brought in to finish drilling the vertical section, run/cement 9.625 casing, and continue onto the lateral.

Precision Drilling Rig #110 began process of walking to P#29761 this morning. BOP testing will commence this evening.
1463403971 2016-05-13 Drill / Deepen / Reopen No Violations On location to witness stimulation operations for this well. PDC is having a zipper frac done and is using a diverter to switch back and forth between the Mason 2-H and 4-H between frac stages. Witnessed the pumping of stage #7. Average treatment pressure was 7430 psi with a rate of 80.0 bbl/min. The initial shut in pressure was 4483 psi and the 5 minute shut in pressure was 3794 psi. Total proppant for the stage was 399588 lbs.
1543570392 2015-11-05 Casing String Witnessed No Violations On location to witness the cementing of the 20 surface casing for this well. Universal Well Services is the cementing contractor for the well. Hall Drilling is the drilling contractor. 20 casing was set to 534 inside of 548 of 24 hole. Universal pumped 195bbls of freshwater ahead, 5 bbls of gel spacer to condition the hole, circulation was established, and 155 bbls of type 1 cement was pumped followed by 185 bbls of freshwater displacement. Good cement returns of 36 bbls were observed at the half round circulation tank. PDC Energy will wait on cement for 8 hours before bleeding pressure off of the cement head and pressure testing the casing.
1569903860 2015-09-16 Administrative Inspection No Violations Received call from Mr. Don Kreager of PDC Energy stating that, after spudding the 34 conductor hole for this well, the hole caved in at 12 deep due to the presence unconsolidated sand and gravel. It was decided to allow PDC Energy to use 30 drive pipe on the condition that the pipe is landed in competent bedrock. I instructed Mr. Kreager to make sure the rig crew is vigilant while drilling and is watching out for potential washout on the backside of the drive pipe, which may affect rig stability as well as hole quality.
1698210096 2016-02-01 Drill / Deepen / Reopen No Violations On location to check status of drilling operations. Received cement tickets and bond log for 2-stage 9.625 deep intermediate cement job. Cement top indicated on bond log at 1220. 9.625 casing depth is 5993. I spoke to PDC Company Man Larry Howell, who stated Schlumberger directional tools are currently in the hole 20 off bottom. Drilling of the 8.75 lateral section will commence after pits are cleaned and prepared for switching to oil based drilling mud.
1736348282 2015-09-25 Drill / Deepen / Reopen No Violations On location to check status of 30 drive pipe drilling. HAD Drilling Contractors were picking up a 28.75 air hammer bit in preparation for re-welding the next joint of drive pipe to continue drilling. Drive pipe is currently at 25 in depth and is still in sand/gravel. Work will continue through the weekend until the pipe is sufficiently landed in competent bedrock.

Mr. Dave Gray of PDC Energy stated that the cementing of the 13.375 intermediate string went well. There is 1670 of 13.375 casing cemented to surface inside of 17.5 hole. The current 12.5 hole section is at 2133 and drilling at an average ROP of 85 ft/hr. Hall Drilling is drilling with a 12.5 air hammer bit.

1977330638 2018-11-27 Production Wells No Violations On this day, I am accompanied by ODNR Southwest Supervisor Eric Conrad. Utica Resources Operator, Jimmy Reese, was on location. The site consists of (2) producing wells and (2) gas processing units on a gated pad within an earthen dike. The production facility is off the pad, located near the gate, and consists of (4) 400 barrel steel tanks and (2) 500 barrel steel tanks all within steel containment with a liner. Annular pressure monitoring devices are equipped and labeled on each wellhead. The 13.375 inch diameter casing gauge has a pressure of 220 pounds per square inch, and the 9.625 inch diameter casing gauge has a pressure of 0 pounds per square inch. Fencing encompasses the wellheads, and proper identification was requested to be placed on the individual wells.
2031896262 2016-02-08 Administrative Inspection No Violations Spoke over the phone at 7:43 am with PDC Company Man Dave Gray. Mr. Gray stated that current bit depth is 13,162 and he estimates TD of 13,820 will be reached later today. Azimuth is currently 1.08 degrees and inclination is 89.44 degrees.
2062831943 2019-05-14 Production Wells No Violations On this day, I found the pad gated with signage. There are (2) producing wells, (2) gas processing units, (2) 500 barrel steel tanks, and (4) 400 barrel steel tanks in containment. The wells are surrounded by livestock fencing and have annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 0 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 220 psi. The production records are current. It will be noted that I spoke with Utica Resources Representative Ryan Weisenbarger onsite.
