0044582282 2015-06-15 Drill / Deepen / Reopen No Violations Met with Flat Rock Development Company Man Mr. Daniel Ratliff to receive cement tickets from Clinton plug and 4.5 production casing cement job. Whipstock Drilling is rigging down and the rig will be moved off location over the next few days. Solidification of drill cuttings and salt mud is taking place on location in preparation for disposal at an EPA approved landfill. The location will be prepped for completion operations starting next week.
0292889779 2016-06-21 Preliminary Restoration No Violations On location with SW Region Supervisor Dan Goins and Engineer Tech Andy Thomas to evaluate preliminary restoration efforts for compliance. Upon inspection, it was observed that the location is still in significant noncompliance with ORC 1509.072. Work is in progress to repaird severe slips along the lease road and well pad area. Slips can be observed opening back up along recently repaired areas of the lease road, indicating remedial work has been inadequate. In addition, erosion and sedimentation controls which are in place along the creek flowing through the location are buried in sediment and are no longer effective. Significant erosion and sedimentation is occurring in several areas throughout the well location. A request for NOMSV will be submitted in regard to significant violations at the well location. New photos were taken to support the findings of this inspection.
0470620029 2015-11-25 Production Wells No Violations Upon inspection, it was observed that this well is still in the flowback phase after stimulation, but is also selling gas. A roust crew from Heinrich Enterprises Inc. was on location flowing fluid off of the well into 4 300 bbl frac tanks until it would build gas pressure, at which time they would divert the flow through a choke manifold and over to the production tank battery. The tank battery consists of 2 100 bbl production tanks, a mechanical separator, drip, meter run, and containment dike. The location has been re-done to reduce erosion potential. The pit has been filled and put back to a 3-1 slope, and previously identified slip areas have been feathered out into a 3-1 slope. I will continue to monitor the restoration progress of this location.
0473457675 2015-04-23 Drill / Deepen / Reopen Violations Noted This notice is to acknowledge that Flat Rock Development, LLC failed to notify ODNR DOGRM Inspector L.J. Parkins of commencement of drilling operations, installation of conductor casing, and cementing of said casing in the wellbore. According to Mr. Scott Gannon of Flat Rock Development, LLC, Lightning Energy Services spudded the well on Saturday, 4/18/2015 with a 20 bit and cemented a 100 string of 16 conductor casing in the well using Allied Cementing Co. as the cementing contractor.

ODNR DOGRM Well Permits specify that the Oil and Gas Inspector must be notified at least 24 hours prior to: 1. Commencement of site construction 2. Pit excavation and closure 3. Commencement of drilling, reopening, converting, or plugback operations 4. Installation and cementing of all casing strings 5. BOP testing 6. Well stimulation 7. Plugging operations 8. Well pad construction This notice will serve as a warning that further failure to notify the Oil and Gas Inspector of any of the above operations may result in suspension or shutdown of drilling operations.

0543156035 2015-05-26 Drill / Deepen / Reopen No Violations On location to check status of drilling operations for this well. Flat Rock Company Man Mr. Daniel Ratliff stated that the TVD is currently 7393 and they are currently tripping a bit into the hole to ream the well bore back to gauge due to the 7.875 air hammer being undergauge after tripping it OOH. Mr. Ratliff estimated that coring will commence next week.
0563491214 2016-06-10 Preliminary Restoration No Violations On location to check status of preliminary restoration violations. Contractor Jeff Jackson is on location with a bulldozer and excavator and is driving sections of 4.5 casing into the fill slope below the lease road in an attempt to stabilize the soil and prevent further slippage. A drainage pipe has been installed to get rid of groundwater, which is playing a large role in lease road slippage. I will continue to monitor the progress of this location toward compliance.
0616664137 2016-05-16 Preliminary Restoration No Violations Upon inspection, it was observed that a new slip has presented itself along the lease road. The slip has moved approximately 2.5 and has damaged the lease road. No work has been done as of yet to show progress toward compliance with preliminary restoration requirements. I spoke with Scott Gannon of Flat Rock Development LLC and notified him of the new slip. Mr. Gannon stated he would speak with their contractor to discuss an approach to fixing the slips in the road and addressing other erosion and sedimentation issues on the location.

Sam Johnson of Heinrich Enterprises states that dirt work will begin soon to address slipping issues in the lease road and other erosion and sedimentation issues throughout the location.
0937908386 2016-01-06 Production Wells No Violations Upon inspection, it was observed that this well is in production. A heated choke manifold is still in place to keep flowback water from freezing off and stopping gas production. Mr. Sam Johnson of Condevco was on location monitoring the choke manifold. Crews are in the process of moving frac tanks off location to allow for preliminary restoration activities to continue. I was accompanied on this inspection by DOGRM Hydrogeologist Jake Glascock.
1020593186 2016-10-20 Production Wells No Violations Compliance Notice Follow-up inspection, accompanied by DOGRM Eng. Tech. Andy Thomas and Flat Rock Dev. Representative Scott Gannon: Upon inspection, it is observed that vegetation is now growing well in previously hydro-seeded areas along the lease road and well location. Several barren areas still exist and will need to be re-seeded to prevent soil erosion. Silt fencing along the waterway running through the location is compromised in areas and needs to be repaired or replaced. Several rills and gullies have formed above the waterway, downgradient of the closed pit area where channelized runoff has occurred. Mr. Gannon verbally agrees to have barren areas re-seeded, to repair silt fencing as needed, and to have all rills and gullies smoothed out to prevent further erosion. Mr. Gannon also states that the 4 400 barrel flowback tanks will be pumped down and removed from the location within the next month. Location is not yet in compliance with preliminary restoration requirements.
1060481464 2015-07-21 Administrative Inspection No Violations I spoke over the phone at 8:30 am with Mr. Daniel Ratliff of Flat Rock Development. Mr. Ratliff stated that the frac job for this well has been postponed and rescheduled for mid-September. Erosion and sedimentation issues on the location are being addressed this week as there is less rain in the forecast.
1075124551 2015-09-03 Drill / Deepen / Reopen No Violations Performed an inspection on this well pad with Flat Rock Development representative Scott Gannon to evaluate options for decreasing sediment-laden runoff from the location. Upon inspection, it was observed that bull dozer work has been done along the lease road to dress up rills and gullies formed by heavy rains earlier this summer. The slope along the lease road has been made more gradual to encourage better stability. I recommended to Mr. Gannon that the drainage ditch along the lease road be lined with rock to dissipate energy from stormwater runoff. It was also recommended that sheet pilings or another form of slope retention be installed at the base of the lease road slope to mitigate slipping issues. Lastly, I recommended that the slipping section along the south-western edge of the well pad itself be taken to at least a 3:1 slope to encourage better stability. The well is scheduled to be fraced on September 18th, after which date all equipment will be removed from the location and preliminary restoration activities will commence..
1257883225 2015-05-21 Drill / Deepen / Reopen No Violations On location to check the status of drilling operations for this well. Flat Rock Development Company Man Mr. Daniel Ratliff stated that Whipstock Rig #1 is currently drilling at 4055 with an 8 hammer bit. Mr. Ratliff anticipates coring will commence mid-week next week.
1280773198 2015-05-12 Drill / Deepen / Reopen No Violations On location to witness the cementing of the 11.75 surface casing for this well. Upon arrival, Universal Well Services was on location and rigged up to begin pumping. Circulation was achieved after pumping 108 bbls of freshwater. 20 bbls of 6% gel with LCM was then pumped ahead of the cement to condition the hole. A total of 116 bbls of class A cement slurry was spotted in the well by 5:28 pm. The wiper plug was sent down at 5:30 pm followed by 102 bbls of freshwater displacement. Cement returns were seen in the cellar at 90 bbls into displacement. An estimated 14 bbls of cement returns were seen at the surface. Upon landing the wiper plug, the plug was bumped, pressured to 1000 psi, and held for 5 minutes. The pressure intitially bled off to 985 psi and held steady, indicating a successful pressure test. After bleeding pressure off of the plug, water began to return to the pump trucks and did not stop after 10 minutes, indicating the float collars may be leaking. It was then decided to lock in 70 psi on the cement head to keep the floats shut until the cement sets up. Cement ticket was received before departure.
1325571194 2015-07-08 Drill / Deepen / Reopen No Violations Inspected ephemeral stream leading from this location in response to a complaint from a concerned downstream landowner, Mr. Dave Edgar. Upon inspection, it was observed that a large amount of sediment-laden water is discharging from this well location. I will be meeting with DOGRM Eng. Tech. Andy Thomas tomorrow to inspect the well location and determine what action needs to be taken by the operator to lessen sediment-laden runoff from the location.
1334631429 2016-03-24 Preliminary Restoration Violations Noted On location to inspect the preliminary restoration of this well location. Upon inspection, it was observed that there are several slips along areas of the lease road which were built on or along an unstable fill slope. There has been over 1 of movement in some areas along the lease road. There are also several areas above the lease road and along sloped areas which are void of vegetation and there is significant erosion and sedimentation occurring. Silt fencing along the ephemoral stream running through the location has been damaged in areas and is allowing sediment to enter the stream. Photos were taken to support the findings of this inspection. Flat Rock Development LLC has failed to meet preliminary restoration requirements as per ORC .072A. Flat Rock Development LLC will have until 6/1/2016 to have the following accomplished: 1.) Remediate all slips along the lease road; 2.) Repair or replace silt fencing which is damaged to keep sediment out of the stream; 3.) Re-seed and mulch areas void of vegetation.
1342304991 2016-04-11 Production Wells No Violations On location with Scott Gannon of Flat Rock Development and equipment operator Jeff Jackson to discuss preliminary restoration compliance requirements. It was explained that the slipping areas along the lease road must be remediated and will likely need to be held back with pilings. It was also explained that all barren areas must be seeded and mulched to encourage vegetation growth. I will continue to monitor this location for compliance with preliminary restoration requirements.

1415983018 2015-05-04 Drill / Deepen / Reopen No Violations I spoke with Flat Rock Development Company Man Mr. Daniel Ratliff in person about the compliance notice issued for failure to notify the division of spudding the well and cementing conductor casing. Mr. Ratliff stated that he and the rest of Flat Rock Developments crew now understand that inspector notification must be given 24 hours in advance of all of the instances listed on the ODNR DOGRM Well Permit. The compliance notice dated 4/23/2015 can be closed out.
1467828935 2016-07-05 Preliminary Restoration No Violations On location with DOGRM Engineer Tech Andy Thomas to inspect this location for compliance with preliminary restoration requirements. Upon inspection, it was observed that efforts to stabilize slip areas along the lease road have been inadequate. Rain events have caused the recently repaired fill slopes along the lease road to begin slipping again. Substantial erosion and sedimentation is occurring in several areas throughout the location. Erosion and sedimentation controls for the location have been rendered ineffective by sediment laden runoff. No seed or mulch has been applied to barren areas throughout the location. Drainage culverts along the lease road and at the NW corner of the location are not functioning properly due to heavy sediment inflow. New photos were taken. Andy Thomas has made contact with Scott Gannon of Flat Rock Development LLC and Mr. Gannon stated intent to contract a third party consulting firm to draft engineered plans to submit to the division addressing preliminary restoration requirements.
1476207865 2016-09-27 Preliminary Restoration No Violations On location with DOGRM Engineer Tech Andy Thomas to evaluate this location for compliance with preliminary restoration requirements. Upon inspection, it is observed that slip remediation work has been done along the downslope of the lease road. The slip remediation work consists of a trench along the toe of the slope which has been filled with large rip-rap rock to help stabilize the slipping material. Buried sections of 4.5 diameter pipe from previous slip remediation attempts remain sticking out of the slope. The open sections of 4.5 diameter pipe create a conduit for stormwater to run into and create wet areas, which may further destabilize the slope. Barren areas around the location have been hyrdoseeded, however very little growth has been established due to minimal precipitation and poor soil quality. Silt fencing along the stream is no longer effective due to sediment buildup from stormwater runoff. The following activities will be recommended to Flat Rock Development in order to reach compliance: 1.) Remove all exposed sections of open 4.5 diameter pipe to help prevent further destabilization of fill material on slope; 2.) Re-seed, fertilize, and mulch all barren areas to encourage vegetative growth; 3.) Repair and/or replace all compromised silt fencing along the stream to help prevent further sedimentation of the stream.
1625975861 2015-06-09 Drill / Deepen / Reopen No Violations On location to witness plug back operations for this well. Upon arrival, Universal Cementing was rigged up and circulating on the well bore to clean out any residual cuttings and entrained gas before pumping the bottom plug. A 550 bottom plug was then set from 9450-8900 using 153 sks of class A cement and 75 bbls of freshwater displacement. The 2.875 tubing was then pulled out of the cement. Whipstocks crew will wait on cement for 8 hours and tag the plug before the Clinton Sand. Plug is set this evening. Cement ticket was received.
1860666328 2015-09-18 Drill / Deepen / Reopen No Violations On location to witness 6000 bbl frac job for this well. Producers Service Corp. was the contractor for the frac. The Marcellus Shale broke down at 1934 psi and an injection rate of 65 bbl/minute at 3000 psi was established. 100 mesh, 40/70 mesh, and 30/50 mesh sand were used. The initial shut in pressure was 1466 psi, 1450 psi after 1 minute, and 1400 psi after 5 minutes. The well will be flowed-back on Monday 9/21/2015. I was accompanied on this inspection by Inspector Brian Sefcik.
1968603336 2015-07-09 Drill / Deepen / Reopen No Violations On location with DOGRM Eng. Tech. Andy Thomas in response to a complaint of over-sedimentation of nearby ephemeral streams leading from this location. Upon inspection, it was observed that the area along the southern edge of the lease road is slipping and the loose overburden material used to fill the area below the road is becoming rilled out and causing significant erosion problems. In addition, the SE facing corner of the well pad is slipping and eroding away, contributing to sediment runoff problems. The existing silt fencing around the location has been compromised and needs repaired or replaced. Additional erosion and sediment controls will likely need to be implemented to remedy excessive sediment runoff from the location. I will contact Flat Rock Development and and set up a meeting on location to make recommendations on how to effectively address erosion and slipping issues. 12 frac tanks are currently on location and the well is tentatively scheduled to be stimulated on July 20th.
1987089330 2015-06-04 Drill / Deepen / Reopen No Violations On location to witness preparation of core samples from the Utica-Point Pleasant Formation. The samples were pulled from 9180-9300. Samples were prepared for ODNR DOGRM as requested in the permit. I was accompanied by ODNR DOGRM Permitting Geologist Mr. Rick Cisler.
2053559241 2015-05-04 Drill / Deepen / Reopen No Violations Met on location with Flat Rock Development Company Man Mr. Daniel Ratliff to assess changes made to the location. Loose overburden and trees were removed from a highwall near the rig footprint to avoid possible collapse during air hammer drilling. The cuttings pit is constructed and will be lined today. The 8.625 cuttings blow line will be run across the creek/drainage inside an 11.75 encasement to help ensure that the creek is protected in the event that the blow line is compromised. Silt fencing throughout the location is compromised due to rain events and constant dirt work being done. Mr. Ratliff stated that Super Silt Fence will be installed to provide greater protection against siltation of the creek. Whipstock Drillings Rig #1 is currently being assembled on the location. Mr. Ratliff estimated that drilling of the surface hole would commence later this week.
2088836693 2015-05-11 Drill / Deepen / Reopen No Violations On location to check status of drilling operations. Whipstock Rig #1 is on location for the drilling of this well. The surface hole has been TDd at 921, but due to a failed mud/soap line, the rig had to be shut down for repair. After fixing the line, the drill string was found to be hung in the hole. A wireline truck was brought onto location and located the free point at 856. An 80g primer cord shot was used to thump the collar at 856 to assist the rig in twisting the collar loose. The shot may have freed the entire assembly as circulation and air rate was reestablished and the string weight remained the same as before the drill string was hung. Flat Rock Development Company Man Daniel Ratliff stated that if the entire assembly comes out then he will have the driller pick up a 14.75 bit and run back to TD to clean up the hole before running the 11.75 surface casing.
