They were rigged up with Cudd energy services with a large BOP stack from BTI Group and a snubbing unit from Cudd. Warren drilling rig 12 was being used to run pipe. They were tripping 2.87510lb/ft drill pipe in well to drill out and move toward the fault.

Witnessed the bit tripping out of the hole and examined the used bit which had been used for 500 of drilling. Andy Thomas took pictures of the used bit. Toured the well site with Triad Hunter Company Man and Safety Coordinator from Weatherford Drilling.

I reminded him that the rules state that liners must be hung 200 ft. into the previous casing string.

He called to notify me that they will be running a 7.625 in liner in the curve section. I reminded him that the rules state that liners have to be hung 200 vertical feet inside the nest larger casing.
1395076883 2014-03-17 Drill / Deepen / Reopen No Violations Drilling had reached a total depth of 14,258 ft. 14,213 ft. of 5.5 in. casing was ran and cemented on 3/16/14. I picked up paperwork for the cement job and spoke with the company man on site. We also talked about the next step for drilling on this pad which is running down with 3.5 in. tools and drilling out past the 5.5 in casing. I expressed my concern on this matter due to the pattern of activity associate with a fault zone that they are drilling close to. I then called Inspector Pat Shreve and discussed it with him, then I called my Supervisor Dan Goins and Deputy Chief Dave Claus. Deputy Chief Claus told me to get a hold of Engineer for Triad Mr. Tim Neill and get a plan and procedure together for this operation. I spoke with Mr. Neill and he agreed to send me one via e-mail.

I was accompanied on this inspection by DOGRM Emergency Response Coordinator Nathan Lane.

1709028885 2014-02-05 Casing String Witnessed No Violations My inspection finds the rig, Warren Rig 12, ran casing to a depth of 419. Baker Hughes circulated 627 sks of PNE-1 cement into place. I witnessed approximately 20 bbl of cement returns to surface.
1776340646 2014-02-13 Casing String Witnessed No Violations Witnessed the cementing of the 13.375 in. casing string. 1919 ft. of casing was ran in 1970 ft. of 17.5 in. hole. Good circulation was achieved throughout the whole job but no returns were observed. I looked at a sample of cement that was pulled when Baker Hughes started mixing cement. After letting it set for a while, no free water was observed on the top. Since there were no returns at the surface company will run a bond log to find the cement top.
1787084579 2017-12-28 Completion Testing No Violations Status check inspection on the well completion process found Stingray Pressure pumping on location completing stage 34 of 35 to be stimulated. The well bore is being completed in 150 increments and perforated by Kodiak Wireline Service with 5 clusters of 7 shots each cluster. The total sand used is 175,000 pounds per stage with a total treating mix water total of 7,200 barrels. The initial shut in pressure is 3,000 psi. All annulis is being monitored electronically and hooked into a gas buster tank flowing through a pressure control flow valve. The well head is a Universal Well Service 15,000 psi rated working pressure along with all the pressure pumping equipment. All stimulation equipment and support equipment is situated on polymer containment.
1975591368 2018-07-16 Production Wells No Violations Status check inspection found the well now in production. The well is equipped with a lower A section attached to the 13.375 diameter casing string with a riser pipe and both a manual and electronic transducer gauge on the 13.375 well head section, the gauge reads 0 pounds per square inch (psi). The upper B section of the well head has a riser pipe and pressure gauge present monitoring the casing pressure of the 9.625 diameter casing string with 0 psi present on the manual gauge. The 5.5 diameter casing string well head section shows 960 psi and the tubing well head section production flow line shows 586 psi. The well head is equipped with an emergency shut down valve that is controlled both remotely and by multiple electrical emergency shut down sites on the well pad. The well is selling through a sand trap system before entering the gas processing unit followed by a contact tower and glycol heater. The final gas sales point is a Blue Racer midstream sales point on the North side of the well pad. Also present is a production fluid facility consisting of 4 steel construction tanks of 400 barrel fluid volume, the tanks are situated inside a polymer lined steel containment dike area. The company has installed a low pressure vapor recovery system on the production equipment.
