0307238255 2012-02-09 Drill / Deepen / Reopen No Violations I found drill rig on location coming out of the hole due to a plugged bit. They report an injector is going out of the rig engine so it is slow going - they predict, if they can get the engine fixed, that should be ready to cement surface casing late Friday or sometime on Saturday. The drill pit has been properly lined and the water truck is bringing in fresh water for the cementing. The permit is posted in doghouse and silt fences are positioned on the down side of rig to protect the small stream contamination.
0447762035 2012-07-13 Drill / Deepen / Reopen No Violations A random inspection this day found both drilling and plugging pits still open, about to run over with fresh water due to heavy rains earlier in the week. Oil had accumulated in a ditch above the culvert below the drill pit, possibly from frac tank or some other source but about to go into drainage ditch. Built dike to stop it and called Tony of Broad Street - he said would have trucks down right away to drain pits and clean up any oil on the ground. Oil only in patch about 2 long and less than a quart but pit looks like it might have more on top. I or another inspector will check on Monday to make sure all work is completed.
0452514725 2012-04-12 Drill / Deepen / Reopen No Violations Killbarger drill rig shows up 4/9/2012, starts cleaning hole 4/10/2012. This day, they were at 3565 and shut down briefly to mix salt water mud. They did not start to ream hole until 2810 and this day while waiting, ran DV tool to see if hole was not straight. Found it to be 2.5 degrees off at 1800, at 3550/ was zero degrees off. This does not explain why previous service rig had trouble getting down hole between 1500-1900.

Silt fences still up, supported by straw bales but all signs of drilling mud in the shallow pools in the pasture are gone. G & L drill rig is moving off location today, Broad Street will have Knox Energy service rig come in and clean out hole and run production casing. Company water truck on location draining the pits. Took pictures showing no cutting or signs of drilling mud in the stream, also across the road where mud had run into larger creek. Today, I saw no traces of drilling mud or cuttings. Citation is RESOLVED.

I stated the need for a blow out pre-venter and that we should do the test after surface casing is set. The surface casing will go thru the Berea - target injection zone the Clinton/Medina.
1250735796 2012-03-01 Drill / Deepen / Reopen Violations Noted I arrived at drill rig today at 8:15 AM, having been called out to witness cementing of production string of casing. When I arrived, I found they were not ready but had casing in the well to 5500 and could not circulate. Casing was again stuck in the hole. When I did arrive, found small stream that parallels lease road running with drilling mud. I found the drill hole was full of mud and running over onto the surrounding ground and was running down into the small stream below the rig instead of running down the flow ditch from the rig and into the drilling pit. As we arrived the drill crew was quickly trying to shut off the flow of mud, using the dozer to cover and fill in the ditch the mud was flowing downhill to the stream. Bill Stolanick quickly took samples while I went up the rig and took pictures and gathered information as to what had happened. After closing off the ditch, the stream was flowing clean at the end of an hour. I called OEPA, Rocky King, supervisor and starting working on clean-up. Much of the drill mud had crossed the road thru the culvert and was laying in shallow pools in Mr. Flowers cow pasture. I advised him to keep cows out for a few days. In conversation with Trevor Irwin from OEPA, decided to let these pools dry up and work into the ground. He says they do not look at drill mud as a hazardous material. He suggested if there were sediments laying on the stream bed, to collect and dispose of properly. Late in the afternoon, rig owner and Broad Street supervisor still trying to find a way to get casing run to bottom and cemented. Citation will be written - owner needs to clean up area impacted by spilled drill mud, fix spilled pit liner.
1321403211 2012-02-23 Drill / Deepen / Reopen No Violations Total depth of 6177 for this well reached on 2/19/2012. They hit no significant water while drilling the initial 6.5 hole. 2/19 - Open- hole logged well and loggers only got to 5900 - top of Clinton sand was 5800. They came out of the hole, went back in with drill pipe and bit and drilled to 6277 - cleaned hole 2.5 hrs. (with air and soap), came out of hole. 2/20/12 - Again tried to log and only got to 5600. 2/21/12 - went back in with 4.5 casing and drill bit (jets removed) in hopes of getting to bottom and cementing casing - used 300 bbl oil-based mud that was 10.5# weight . Started heavy mud at 5200 - starting 400 above where well got tight. At 5644 lost circulation, pulled out 2 joints to try and get circulation back, twisted off 2/22/12 PM at @5600. Pulled casing and found 3570 with bit left in hole. 2/23/12 - Overshot grapple fishing tool arrived with operator, heavy mud coming in - digging and lining a 2nd pit to mix mud. Have decided to ream hole all the way down with 7 7/8 bit. Went back in fishing tool, grabbed casing and found it free - found it had dropped @ 120 in hole. Will begin again 2/24/12 AM with fresh start to recover 3570 of casing left in hole. Goal is to ream hole to bottom, drilling with heavy mud and LCM additives, clean hole, go back in with casing and cement in place. From 2/22/12 on, well making considerable water and blowing back out of hole at connections - they are still pumping water down other disposal well on property and I strongly suggested they shut it down until this well cemented. They say too costly as have a take or pay contract. To be continued.
1331599379 2012-03-12 Drill / Deepen / Reopen No Violations On 3/8, rig had casing in the hole to 4576 and was starting to mix mud in hopes of getting pipe to bottom and cementing it in place. After I left, they found bit on bottom of casing was plugged and they would have to come out of the hole. They pulled all pipe on 3/8. On Friday, 3/9/12, they decided to run a deviation tool in the hole and this was scheduled for 3/12. I was on location this day to see the logger try to run tool and he got to 5485 which was in the base of the Big Lime. He could not go deeper but this was 1000 deeper than the rig got with casing. The data is being studied and decision will be made soon as to what the next step will be.

2050723671 2012-04-26 Drill / Deepen / Reopen No Violations 4/16/12 - Talked to tool pusher, Gary Smith - he said they were at 4707 this morning and tripping for a bit - will put on a new tri-cone. Had tripped Friday night when they thought their PDC bit was bad and had put on a re-run tri-cone at that time. Still drilling slow. I passed this information on to Dave Ball, the UIC inspector. 4/23/12 - Talked to Ed Butler and he reports rig is at 5945 and still reaming. They should finish this week, clean & condition the hole and then run casing & cement in place. 4/25/12 - Talked with Ed Butler - he says they will plug the hole - wanted to know how we wanted it plugged. I suggested he call Dave Ball as he is UIC inspector. The rig went back in the hole after unplugging the bit and could only get to 5885 - lost almost 100. Later in day talked to Dave Ball and they decided to try all day 4/26/12 and then make a decision on whether to plug.
