1226870619 2016-05-25 Final Restoration No Violations The purpose of my inspection today is a status check on a final restoration. During my inspection I was accompanied by ODNR Inspector LJ Parkins. The inspection revealed that there was no evidence of an oil and gas well and the site has been restored. Location has met 1509.072 B requirements.
1406235457 2014-07-24 Plug / Plug Back No Violations Visited this location to witness the plugging of this well. Upon arrival, Knox Energy was rigged up over the well. Cemenco arrived on location at 9:45 am and set a 400 bottom plug with 30 sks of class A cement and displaced with 12.3 bbl of fresh water. A 40 lb bag of cottonseed hulls was used as LCM. Knox Energy then found telegraph at 462 and attempted to rip the 4.5 casing at that depth. After 42 minutes of attempting to rip, it was decided to make an attempt at backing the pipe off with power tongs. Knox Energy was able to break the casing free at the desired depth. Circulation was established before setting the bottom-of-surface plug. The bottom-of-surface plug was spotted from 360-160 using 40 sks of class A cement. Fluid level was gauged at 100 below surface and stayed at that level for 15 minutes before pumping 20 sks of class A cement to establish top plug from 100 to surface. The cement settled approximately 6 and stayed.
