0110321120 | 2009-04-30 | Production Wells | No Violations | Well pumping this day and wanted to see where water was going - pumps many hours. Water going into tank, has good IDs, no dike. Pit still open but empty from when had service rig on well. Will call Mike, pumper, and ask to get pit filled in. |
0356351145 | 2008-06-10 | Production Wells | No Violations | Ols cable tool rig has been pulled off the hole, well producing gas for landowner, jack sitting off to side yet to be put on well. Gas is bubbling pretty good between 4.5 casing and surface - may have a problem if pressure builds. Good IDs at tank, no dike. |
0760093901 | 2019-05-07 | Production Wells | No Violations | I was back out on location today to witness the plugging of this well. The 5.5 inch casing was parted at 477 feet and pulled from the well. The 2 inch tubing was ran back in the well to set the bottom of the surface plug and the surface plug. The plugs ran together from 409 feet to the surface. The cement held at the surface with zero settling. The well is now plugged. |
0809435051 | 2019-05-02 | Plug / Plug Back | No Violations | I am on location today to cover the plugging activities on this well. Today the rig crew worked on getting the tubing back down the well from where the packer parted the 5 inch diameter casing. They were able to get back down to 1485 feet. They then had Timco Wireline Services come out to shoot off the tubing. Timco tried at 1450 feet at first and the tubing would not pull free. They ended up logging the tubing and found the packer was up hole. We shut down for the day to line up the cement crew. |
0877808741 | 2019-04-23 | Plug / Plug Back | No Violations | I was on location today to check on the plugging activities at this well. Harmon Well Services has moved their service rig in on this well to plug it. They have pulled the 1 inch diameter tubing and are now working on getting back down the 2 inch diameter tubing so they can shoot the pipe off. The tubing is currently hung in the hole. |

1231446588 | 2009-01-07 | Production Wells | No Violations | Harmons service rig on location this day but no crew - very cold and wet - will be trying to release packer set incorrectly in well and get well back in production. Still have working pits open. |
1556904670 | 2019-05-03 | Plug / Plug Back | No Violations | On site inspection to witness the well plugging process accompanied by ODNR Supervisor Eric Conrad. The well bore is equipped with a 14.5 diameter conductor along with a 9.5 out side diameter surface casing string followed by a 5.5 diameter production casing string. Also present is a 2.375 diameter production tubing string, the 2.375 diameter production tubing string length was marked at 1,480 with a packer set in the string at 980, the packer was marked by logging through the means of Timco s wire line service. The 2.375 was 1st shot off at 1,450 and a 50 sack class A neat cement plug was pumped through the 2.375 string by Formation Cementing. Timco then ran back in the hole and tagged the top of the cement plug at 1,180 and shot the 2.375 diameter tubing off at 950. The tubing was then pulled out of the well. |
1582345122 | 2018-11-21 | Production Wells | Violations Noted | I was on location today to do a field inspection on this well. During my inspection I found the well is not capable of any production. All of the production valves on the wellhead are currently closed making the well incapable of production. The production lines from the wellhead to the tank pad are no longer connected. The last time production was reported for this well was in 2010. The well owner/operator shall submit a written plugging schedule to (Justin Smith) at (5880 Memory Rd Zanesville Oh, 43701) within 30 days, per ORC 1509:13(B)(7), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied. |
1646630364 | 2019-05-01 | Plug / Plug Back | No Violations | I was unable to cover this job today because I was on another plug job. Troy Harmon contacted me later in the day to let me know how the job was going. They were still trying to fish out the tubing today with a fishing tool. While pulling on the tubing they believe the 5.5 inch diameter casing ended up parting and everything is moving. They shut down for the day to come up with a plan to finish this well. |
1824990512 | 2004-05-05 | Preliminary Restoration | No Violations | Inspection this day found cable tool rig still on location, no truck or motor to remove, location has reseeded and good grass, well has jack and is tied into line, tank has good IDs, no dike - needs NOV as has not removed drilling equipment, right Joe? |
1946473450 | 2008-09-02 | Production Wells | No Violations | Inspection this day found service rig on location. Trying to pull a packer that they had run several weeks ago but no luck. Well would not pump. Had tried to drill out packer, were going to PA this day to get something special to help get this well back to producing. Well has jack, tank with good IDs, no dike. |
