0144218836 2014-09-17 Plug / Plug Back No Violations Visited this location to check status of prep work for plugging activities. Timco Well Services was on location to shoot 2.385 tubing off below and above the tension-set packer assembly. Bishop Well Services will pull the 2.385 tubing after it is shot, check it for holes, and run tubing back in the well to set the bottom plug through. Accompanied on inspection by ODNR DOGRM Engineering Tech. Andy Thomas.
0169312865 2014-09-18 Plug / Plug Back No Violations On location to witness plugging activities for this well. Bishop Well Service is rigged over well and the tubing is at 5346. Timco was on location this morning and perforated the Big Lime with 10 shots from 4518-4526. Formation Cementing set the bottom plug through 2.375 tubing using 42 sks 60/40 poz cement, 50 lbs of calcium chloride, and 18.5 bbls of displacement. Tubing was then pulled up to 4396 in preparation to set the Big Lime plug. The Big Lime plug was set using 81 sks of 60/40 poz cement, 150 lbs of calcium chloride, and 16.7 bbls of displacement. Cement ticket was received upon finishing the Big Lime plug. Plugging operations will resume on Monday 9/22/14.
0289440849 2016-06-16 Final Restoration No Violations The purpose of my inspection today is a status check on a final restoration. During my inspection I witnessed no evidence of an oil and gas well and the site has been restored. Location has met 1509.072 B requirements
1410916777 2014-09-16 Plug / Plug Back No Violations Upon arrival, Bishop Well Service was rigged up over the well and was running new 4.5 casing into the well in an attempt to thread onto casing that had parted from a collar. The attempt was successful. The 2.385 tubing was also parted at 170, so a skirt tool was used to guide the parted tubing into a collar and thread onto it. This attempt was also successful. Tools will be run into the tubing to ensure it is clear and the tubing will be shot off at the packer and pulled. New tubing will be run into the hole to set the bottom plug through.
1411474509 2014-09-22 Plug / Plug Back No Violations Arrived on location at 7:45 am to witness plugging activities for this well. Formation Cementing was on location to finish plugging this well. 1.5 tubing was set at 3620 to set a rip plug where the 4.5 casing was ripped above the Big Lime plug. This plug was set from 3620-3420 using 50 sks of 2% gel cement, 8.6 bbl displacement, 100 lbs calcium chloride, and 20 lbs cottonseed hulls as LCM. Tubing was then pulled to 1729 to set a Berea plug. This plug was set from 1729-1529 using 50 sks of 2 gel cement, 3.8 bbl displacement, 100 lbs calcium chloride, and 20 lbs cottonseed hulls. Tubing was then pulled to 1450 to set a bottom of surface casing plug. This plug was set from 1450-1250 using 51 sks of 2% gel cement, 3.1 bbl displacement, 50 lbs calcium chloride, and 20 lbs cottonseed hulls. Good circulation was established for the bottom of surface plug. Tubing was then pulled to 200, and a 58 sk surface plug was circulated into the well. The cement stayed at the surface.
1514935896 2014-12-24 Plug / Plug Back No Violations Performed inspection on this location to monitor final restoration progress. Upon inspection, it was observed that the lease road has been regraded, water bars have been created to prevent erosion, and a sparse vegetative cover has been established. The former well location has been graded, the pit has been filled, all production equipment has been removed, and there is a fairly thick vegetative cover established. I will check back in the spring to see if vegetation took a stronger hold on the lease road before passing the final restoration.
