0077523000 2016-05-26 Production Wells No Violations Upon inspection, it was observed that no effort has been made by the operator to gain compliance with this well. The well is idle and incapable of production. The well is steadily leaking brine from the 4.5-7 annulus. Vegetation kill around the well has not been addressed. Brine leaking from the well maxed out high range Quantab test strips, indicating chloride levels over 6215 ppm. A business card was left at the Hartline residence requesting they call and supply any information they have about the history of the well. Adams Oil & Gas is the responsible owner. New photos taken.
0423402163 2016-01-04 Production Wells No Violations Upon inspection, it was observed that this well is in significant noncompliance. This well was originally cited for 1509-12(b) on 8-10-2011 and the well has yet to be plugged. The well is still idle and is incapable of production. The packing between the 4.5 production casing and 8.625 surface casing is rotted away and brine is steadily discharging from the annular space, onto the ground surface, and running downhill toward a plowed field which is a short distance from Duck Creek. I field tested the brine with a Hach Quantab titration strip and the chloride level maxed out the test strip, indicating the chloride level is well over 6215 ppm. There is a large vegetation kill area surrounding the well head approximately 30 in diameter and extending downhill in a 2 wide x 40 long flow path. I contacted Doug Adams of Adams Oil & Gas and explained that the contamination needed to be addressed immediately and the well needs to be plugged and abandoned. Mr. Adams stated that he does not own the well and he has paperwork to prove it. I told Mr. Adams that he will need to speak with our Bonding and Surety Section to sort any ownership discrepencies out, but as of now the contamination is his responsibility. VOID THIS NOTICE. A NEW, UPDATED NOTICE WILL BE ISSUED TO ADAMS OIL AND GAS ADDRESSING NEW VIOLATIONS.
0734797945 2016-01-06 Administrative Inspection No Violations Met with Mr. Doug Adams of Adams O&G to discuss contamination issues and ownership issues with this well. Mr. Adams provided documentation showing forfeiture of the Hartline lease. Mr. Adams also provided a well assignment sheet which lists the Richard Hartline Unit #1 as P#25023, which is incorrect as that well belonged to Stonebridge Operating Co. and is now plugged and abandoned. I told Mr. Adams that I would forward the documents over to Erica Freeman in our bonding and surety section. I was accompanied at this meeting by DOGRM Hydrogeologist Jake Glascock.

The inspection this day revealed that this well is equipped with an 8 5/8-inch diameter surface casing, 4 1/2-inch diameter production casing, and 1 1/2-inch diameter production tubing. The valves on the wellhead are closed at the time of inspection. The 2-inch diameter lines running from the well have all been rotted away. Bubbles could be witnessed coming from the 8 5/8-inch diameter surface casing at the time of inspection. Production in RBDMS has not been filed for this well since 2003 from Adams Oil & Gas. There has been no resolution to the issues on this well. Updated pictures were taken during the inspection.

1447626925 2018-07-10 Production Wells No Violations On this day, I am conducting a Follow-up inspection to Chiefs Order 2018-89 per request of Southwest Region Supervisor Dan Goins. I found the well equipped with an 8.625 inch diameter surface casing head, 4.5 inch diameter production casing venting to atmosphere, and 1.5 inch diameter tubing venting to atmosphere. No flowlines are connected to the wellhead. The packing between the 4.5 inch casing and the 8.625 inch casing head has deteriorated, allowing fluid to release from the annular space. Standing fluid with continuous bubbling can be witnessed within the annular space. The 4.5 inch casing appears to have corroded and separated just above the 8.625 inch casing head. Additionally, there is fluid in the immediate vicinity of the wellhead with stressed vegetation. The affected area is approximately 30 feet in diameter around the well, tapering down to a 2 foot wide path that leads downhill to a fence line, at the edge of a field. Duck Creek is approximately 250 feet north of the wellhead. Pictures were taken during the inspection.
1452004763 2016-01-04 Production Wells Violations Noted Upon inspection, it was observed that this well is in significant noncompliance. This well was originally cited for nonproduction on 8-10-2011. The well is still idle and is incapable of production. There are no production lines attached to the well. The packing between the 4.5 production casing and 8.625 surface casing is rotted away and brine is steadily discharging from the annular space, onto the ground surface, and running downhill toward a plowed field which is a short distance from Duck Creek. The brine leak has been occurring for a long enough time that salt crystals have precipitated on the well head. The brine was field tested with a Hach Quantab titration strip and the chloride level maxed out the test strip, indicating the chloride level is well over 6215 ppm. There is a large vegetation kill area surrounding the well head approximately 30 in diameter and extending downhill in a 2 wide x 40 long flow path. Photos were taken to verify findings. The operator, Adams Oil and Gas, must address brine contamination issues on the location immediately. Contaminated soil shall be excavated until diminishing returns of elevated chlorides are detected. All contaminated soil shall be disposed of at an EPA approved landfill. The operator will have until 1-15-2016 to complete cleanup. In addition, the well shall be properly plugged and abandoned by the operator. Adams Oil & Gas shall submit a plugging schedule to Inspector L.J. Parkins within 30 days and plug the well within 180 days. Adams Oil & Gas will be notified if the plugging schedule is approved or denied.

This well was originally cited for nonproduction on 8-10-2011. The well is still idle and is incapable of production. There are no production lines attached to the well. In fact there are pieces of the 2 diameter production line rusting away in the chloride laden mud near the well. The packing between the 4.5 production casing and 8.625 surface casing is rotted away and brine is steadily discharging from the annular space, onto the ground. Bubbles were witnessed coming to the surface in this annular space at the time of this inspection. The brine after it hits the ground, then runs downhill toward a plowed field which is a short distance from Duck Creek. The brine was field tested with a Quantab titration strip and the chloride level maxed out the test strip, indicating the chloride level is well over 6215 ppm. There is a large vegetation kill area surrounding the well head approximately 30 in diameter and extending downhill in a 2 wide x 40 long flow path. This flow then hits a level spot and spreads out in both directions at least 100 feet parallel with the fence. I used Quantabs on this lower part and found chlorides in standing water that was nearly 130 in a straight-line from the well. I found chlorides in standing water all along the fence were the brine from the well fanned out. Photos were taken to verify findings. This inspection is a follow up to Compliance Notice 1452004763.
1501590004 2017-08-01 Production Wells No Violations The inspection this day revealed that this well is equipped with a 8 5/8 inch diameter surface casing, 4 1/2 inch diameter production casing, and 1 1/2 inch diameter production tubing. The valves on the wellhead are closed at the time of inspection. The 2 inch diameter lines running from the well have all been rotted away due to the brine that has been sitting there. Upon a previous visit the 8 5/8 inch diameter casing was bubbling but it was not at the time of inspection today. There is still standing brine in the open 8 5/8 inch diameter casing and the brine scald area where the brine has leaked and ran has not been fixed. There has been no resolution to any of the issues on this well and the well is still in non-compliance.

Today, I found well as before, idle zinc not producing with a 4 x 2 sedge, shut in and no identification. There is a 2 steel line coming out of the ground near the well and the 2 100 bbl tanks have been rolled on their side and stacked out of the way by the farmer. No production has been reported since 1985 and landowner states lease has been cancelled. NOV has not been resolved.

1944766139 2017-04-20 Production Wells No Violations On this day I was accompanied by Supervisor Dan Goins and Eric Conrad, Josh Shiering and Rick Anderson to this well. This well is in the middle of a cow pasture and is equipped with a 4 1/2 inch diameter wellhead. The wellhead is leaking brine from the 4 1/2 inch diameter open hole casing at the surface. Around the wellhead there is a significant brine scald that runs down the hill to the fence line of the pasture to another large scald area located at 39.44855, -81.41611. A field test was run using High Range Quantab on top of the open 4 1/2 inch diameter wellhead and the ground in the area around the wellhead. The fluid maxed out each Quantab which showed the chlorides reached over 6215 parts per million. Bubbles were witnessed leaking from the casing head between the 4 1/2 inch diameter wellhead and the surface casing at the time of inspection. The 2 inch diameter steel production lines are all rotted away and disconnected and the valves on the wellhead are open to the environment. Salt crystals could be observed in the area around the wellhead at the time of inspection. We spoke with the landowner and he informed us that the tanks outside of the fenced in area do belong to that well but most of the metal around the tanks are his and he would move it away from the tanks. Updated pictures were taken at the time of inspection. I called Doug Adams and received no answer and left a message for him asking him to call me back as soon as possible regarding this well.

On April 25 at 9:42 Am Doug Adams called me in regards to my message. I informed Doug Adams we had been out to this well and told him that since he was the owner of this well he would need to clean this well up and repair it. He informed me that there was a mistake with API numbers and he never owned this well. I informed him that we had a Form 7 on file showing that he had taken ownership of this well. He told me he had the same form but the API of the well has been whited out and hand written in on his
