0648363710 2016-06-07 Final Restoration No Violations The purpose of my inspection today is a status check on a final restoration. During my inspection I witnessed no evidence of an oil and gas well and the site has been restored. Location has met 1509.072 B requirements.
1439567629 2015-08-14 Plug / Plug Back No Violations I arrived on location at 11:30 behind CEMENCO. Appalachian well services ran a bond log and as previously discussed with inspectors Lynn Parkins and Pat Shreve the casing was shot off at 515. The casing was stuck and would not pull free. A second bond log revealed that the pipe wasnt completely free until 350. The surface casing was at a depth of 250. After the pipe was determined to be free cement was pumped down the 2 7/8 casing until returns were seen from the surface casing. The 2 7/8 was then pulled, 11 joints, then the casing was filled to surface to account for the displacement of the casing. 120 sacks of cement with 5 sacks of gel and LCM was added. THE WELLS ACTUAL GPS COORDINATES WERE 300 FROM WHAT WAS PLOTTED IN DELORME. THE WELL WAS 45 FROM THE SE CORNER OF THE BUILDING.
