I found tank with good identification, all valves shut at tank and going thru meter. I found a little flowmeter as they call it, and it was registering zero. Mr. Wagner says well has not produced in years altho they occasionally send him a check. The well was set up to flow thru the casing and all valves at the well were also shut. Stonebridge has reported 220 mcf/year for annual production in 2010 and I believe this is false information - they are deliquent in turning in their 2011 information. Mr. Wagner has contacted an attorney to cancel the lease and would like ODNR to wait to write a violation until lease is cancelled. |

Upon arrival, Matheny Well Service was rigged up over the well and Formation Cementing was on location to pump the plug. A 400 bottom plug was set using 30 sks of class A cement and 40 lbs of cellulose as LCM. 3.7 bbl of displacement was used to send the plug down hole. The plug was set through 1.5 tubing. Fluid was seen at the surface after displacement, indicating a potential hole in the 4.5 casing. Matheny Well Service will trip out the tubing, find free casing and rip the casing. The 4.5 casing will then be completely pulled and 1.5 tubing will be tripped back in to set the rest of the plugs for this well. |

Matheney Well Service had ripped the 4.5 casing at 1019, pulled the old casing, and ran 1.5 tubing to 1014. After breaking circulation, a 200 rip plug was set by Formation Cementing using 40 sks class A cement, 150 lbs calcium chloride, and 20 lbs cottonseed hulls as LCM. Tubing was then pulled out to 690 and 2.5 hours were given to allow the rip plug to set up. Tubing was run back into the hole to 811 to set a 220 bottom-of-surface plug. After breaking circulation, the bottom-of-surface plug was set using 26 sks of class A cement. Tubing was then pulled to 203, circulation was established, and 40 sks of class A cement was pumped to set a 200 surface plug. The cement settled at 30 below grade and stayed. |
