0010484857 2017-07-18 Production Wells Violations Noted The inspection this day was to check on a previous violation and that violation has been resolved. While I was inspecting this well it was revealed that there was an oil spill near the wellhead. There is a 3 inch diameter plastic line that leads to the tank battery at the bottom of the hill that appears to have possibly broke and has been repaired. The spill goes down the hill a bit and stops on flat ground and covers about 90 square feet. I called Larrey Martin and received no answer. I left him a voicemail informing him of the spill I found and asked him to call me back as soon as possible to discuss the clean up, disposal, and remediation of the area in a proper manner. The well operator shall clean up all contaminated soil.

0641947508 2016-03-04 Production Wells No Violations Performed inspection of the tank battery for this lease due to spotting a brine scald area below the containment dike walls from the road. Upon inspection, it was evident that illegal brine disposal is taking place on the lease. There is a large scald area where vegetation no longer grows and the trees are dead and/or stressed. The affected area extends downhill to the banks of Archers Fork Creek. The release appears to take place at the load valve on the rear of the 100 bbl production tank, where it flows into the tank pad area, into a PVC drain pipe, and onto the soil alongside Archers Fork Creek. Field testing was performed on fluid laying in the tank pad using Hach Quantab chloride titration strips. The fluid in the tank pad maxed out the titration strips, which indicate a chloride level of over 6215 ppm. The brine disposal appears to be intentional, as there are no leaks present at any point on the tank that would indicate an unintentional release. I have contacted ODNR DOGRM Geologist Jake Glascock and set up a field sampling and investigation event for this lease on Tuesday March 8th 2016.
0714973348 2016-03-31 Administrative Inspection No Violations Soil sampling results arrived today from the sampling event on 3-8-16. All samples from the affected area yielded highly elevated chloride levels. The control sample yielded chloride levels less than 12 mg/kg.
0748120219 2016-03-08 Production Wells Violations Noted Upon inspection, it was observed that this well is currently in production. The well consists of a pumping unit standing over 6.625 surface casing, 4.875 production casing, and 2.375 production tubing. The well is producing on the 2.375 tubing and is venting gas on the 4.875 casing. There is an old horizontal production tank along with 3 old plastic knockout tanks which are no longer in use. The knockout tanks have oily fluid in them and need to be removed from the location for proper disposal. There is an old brine disposal pit below the horizontal tank which has old brine/oil contamination in it. The disposal pit must be remediated and closed. The current tank battery for the well consists of a 100 bbl production tank inside an earthen dike. It is evident that illegal brine disposal is/has been taking place at the tank pad. There is a large scalded area, which originates at the load valve on the rear of the tank, and extends downhill from the dike drain pipe to the soil along Archers Fork Creek. The scald area is 30 x 40 at its largest point, and narrows to a drainage ditch averaging 5 wide x 60 long. The ditch flows toward Archers Fork Creek. Soil samples were taken by DOGRM Inspector L.J. Parkins and DOGRM Geologist Jake Glascock. The samples were delivered to the ODNR Environmental Lab in Cambridge for analysis to determine the extent of contamination. Photos were taken to support the findings of this inspection. The operator, Larrey C. Martin, will have 30 days upon receipt of this notice to perform ALL of the following: 1.) Excavate all contaminated soil inside and outside the tank pad and dispose of it at an OEPA approved landfill; 2.) Bring in clean soil to replace contaminated soil, bring location back to grade, lime/fertilize, and seed/mulch the soil to encourage revegetation; 3.) Rebuild containment dike to SPCC guidelines; 4.) Remediate and close old brine disposal pit near well and remove all old knockout tanks for proper disposal.
0799749135 2016-03-30 Administrative Inspection No Violations Spoke over the phone with Larrey Martin at 8:20 pm on 3/30/16. Mr. Martin stated the cleanup operations are currently at a standstill because he is waiting on results from a soil profile from the State of West Virginia before the Wetzel County Landfill will accept the contaminated soil. According to Mr. Martin, cleanup will resume after the current material is disposed of.
1280090072 2016-03-09 Production Wells No Violations On location with DOGRM Geologist Jake Glascock to meet with well owner Larry Martin to discuss brine contamination cleanup options and time frames for cleanup. Upon arrival, Mr. Martin had begun spreading powdered agricultural lime and hay over the scalded area in an attempt to clean up the contamination. I stopped Mr. Martin and informed him that the soil will have to be excavated and properly disposed of an an OEPA approved landfill. I exlained to Mr. Martin that the top 4-6 of soil shall be excavated inside the tank pad area and the entire extent of scalded area outside the dike leading toward the creek. It was also explained that clean soil must be brought in to replace the contaminated soil. The area must be limed/fertilized, and seeded/mulched to encourage revegetation. I explained that the old brine disposal pit must be remediated and closed and all of the old knockout tanks, which are full of oily fluid, must be removed from the location for proper disposal. Lastly, I informed Mr. Martin that this case will be referred to our legal section for enforcement consideration. Mr. Martin verbally agreed to comply with the cleanup requirements.

1408246946 2017-07-18 Production Wells No Violations Inspection this day revealed that this well is equipped with a pumping unit that was pumping at the time of inspection, a 4 1/2 inch diameter production casing, and 1 1/2 inch diameter production tubing. The valves are open on the wellhead at the time of inspection. The tank battery for this well is equipped with a 100 barrel steel tank and a 50 barrel plastic tank. The previous spill reported by Inspector LJ Parkins has been cleaned up and the area has been remediated. There are no issues to report at this time and the well is in production at the time of inspection.
1447312953 2004-12-29 Production Wells No Violations Inspection this day found well not in production, well has jack, could be capable but meter shut off. This well reportedly is owned by Mr. Cook who is selling it to Larrey Martin, well is on WhippleRun/Hugh Dales bond but he doesnt own. Anyway, this well not producing but will be shortly so will give a little time to get in production. No production in many years. - Meter off since 1996? - other shallow wells in area and central pumphouse on property - need to find out whos those wells are. Tank says Cook #2 but believe all 3 older wells going into it.

The valves are open on the wellhead at the time of inspection. The tank battery for this well is equipped with a 100 barrel steel tank and a 50 barrel plastic tank. The well was pumping at the time of inspection. The spill that was found at my last inspection appears to be cleaned up and no sign of oil can be found on the ground. This well is now in compliance.
1690495641 2016-05-03 Production Wells No Violations Upon inspection, it was observed that all contaminated soil has been removed from the location, the previously affected areas have been re-graded, limed, seeded, and mulched. Grass is beginning to grown on the previously affected areas. A new tank location has been established and consists of a 100 bbl production tank, small knockout tank, and a 2100 gallon plastic brine tank. A containment dike was built around the tanks which meets SPCC guidelines. All old knockout tanks, production tanks, and flow lines have been removed from the location. The old brine disposal pit has been closed and the area has been re-graded, limed, seeded, and mulched. Photos were taken. I will continue to monitor the progress of this location toward compliance.

There is still an area leading toward the creek bank which needs to be excavated for disposal. A 2100 gallon plastic tank has been brought onto the location which will likely be used as a brine storage tank. Larrey Martins bulldozer and excavator are still on location. Photos were taken.
2110257999 2016-03-16 Production Wells No Violations Cleanup operations have begun at and around the tank pad area for this lease. The 100 bbl production tank has been moved off to the side. The top 4-6 of soil has been removed from inside the tank pad, containment dike, and main brine scald area. Larrey Martins bulldozer and excavator are on location. No work is being done today due to soft ground from heavy rains the past few days. More soil will have to be excavated along the scalded ditch area leading toward Archers Fork Creek. New photos were taken. I will continue to monitor the progress of this cleanup.
