0347522495 2013-05-23 Production Wells No Violations Inspected well for landowner Chad Walters who has bought this and 4 other wells from Rich Markey. The well has 4.5 in casing with a swage and a valve. There is no tank battery for this well at this time. Mr. Walters told me his plan to get the well back in production.
1832352353 2014-07-17 Exempt Well Pre Transfer No Violations Inspected this well in response to a request from ODNR DOGRM Surety Section. Upon inspection, it was observed that this well is incapable of production. The well consists of 12 conductor and 4.5 casing with a swedge to a 1 valve. The valve is in the closed position and no lines are attached to the well. There is a 100 bbl production tank approximately 100 from the well that likely belonged to this well at one time, but is not connected to any lines. There is no identification at the well location. There are no contamination issues at the well or tank. The tank battery for this well consists of (1) 100 bbl and (1) 210 bbl production tank inside a containment dike. This tank battery is shared with 3 other wells on the property. I spoke with landowner Ms. Evelyn Walters and she stated that she is attempting to gain status as an exempt domestic well owner, and will service the well if and when she is granted such status.
