1744662953 2013-05-23 Production Wells No Violations Inspected well for landowner Chad Walters who has bought this and 4 other wells from Rich Marquis. Well is equipped for production. I told Mr. Walters that the well needs proper identification on it.
2113120526 2014-07-18 Exempt Well Pre Transfer No Violations Inspected this well in response to a request from ODNR DOGRM Surety Section. Upon inspection, it was observed that this well is capable of production. There is a pumping unit equipped with a gasoline motor for this well. The well head consists of 7 surface casing, 4.5 intermediate casing, and 2 production tubing. The production tubing connection is open and has no valve. There is a 1 poly line hooked up to the annular connection on the well head, and is currently shut in. There is no identification at the well location. There are no contamination issues at the well. This well shares a tank battery with 3 other wells on the property. I spoke with landowner Ms. Evelyn Walters and she stated that she is attempting to gain status as an exempt domestic well owner, and will service all 5 wells on her property for domestic use if and when such status is granted.
