1071852641 2015-05-18 Production Wells No Violations Inspected this well in response to a request from landowner Mr. Justin DeLagrange. Mr. DeLagrange states that he owns this well and paid to have it serviced recently by Dave Kale. The well consists of a pumping unit standing over 10 surface casing, 4.5 intermediate casing, and 2 production tubing. Mr. DeLagrange also stated that he is interested in using this well for domestic gas purposes. I explained the process of applying for a domestic owner number and gave him contact information for ODNR DOGRM Bonding and Surety Section. I will return to this well to get accurate GPS coordinates.
1269626827 2015-12-03 Exempt Well Pre Transfer No Violations Upon inspection, it was observed that this well is in production of domestic gas only. The well consists of a pumping unit standing over 10 surface casing, 4.5 production casing, and 2.375 production tubing. The well has been pumped recently. The well is producing gas for domestic purposes on the 4.5 casing, which leads to a regulator, drip, and 1.25 black plastic flow line which leads to a gas orifice where it enters the De La Grange residence. There is a newly installed 1.25 black plastic flow line leading to a new 100 bbl production tank, which sits within an earthen containment dike. There is no ID information posted at the well or tank. I explained to Mr. De La Grange what type of ID information needed to be posted and he agreed to do so. No significant violations that would prevent the transfer of ownership for domestic purposes were noted. *Note: The RBDMS provided coordinates for the well plot on the next property over and are incorrect, the correct coordinates are correct. This was verified by comparing the original permit plat map showing the wells location and parcel data from the Washington County Auditors Office website.
1671309114 2015-05-26 Production Wells No Violations Met with Mr. Justin Delagrange to discuss tank spacing requirements, gas line burial, and SPCC guidelines. Mr. Delagrange is in the process of filing for a domestic owner number with the division.
