0354922308 2009-10-01 Production Wells No Violations Met with Parks and REALM regarding purchase and plugging of well, land use of property. Chief Husted riding with me this day. Talked about cost of plugging, other options for the well and other representatives of ODNR negotiated price for buying the well from Mr. Art Hunter, also present for the meeting. Parks needing to lower the elevation of this tract of land they own to lessen flooding around the Lowell area - want well plugged for this land use. Talked about costs of $12,000 to $15,000 to plug the well, how well will be marked when down and the possibility of leaving cement lower in the hole due to land ultimately being lower.
0658932094 2017-05-02 Production Wells No Violations The inspection this day revealed that this well is equipped with a pumping unit, a 4 1/2 inch diameter wellhead, and 1 1/2 inch diameter production tubing. The valves are open on the wellhead at the time of inspection. The tank battery for this well is equipped with a 100 barrel steel tank with identification painted on it and a meter box with a current chart on it. There are no issues to report at this time and this well is in production at the time of inspection.
0834156130 2010-12-13 Administrative Inspection No Violations Talked with Rochelle and Erica in Columbus - neither have gotten any bonding information in for Mr. Brian Wagner nor a Form 7A adding well to his bond. They said he does have the $1 million insurance - has had for some time. When I talked with Brian on 12/9/2010, he said Columbus had all the information and I suggested to avoid legal action, he contact Columbus and get it straightened out. They said he did not call in last few days, if not weeks.

Crew had been in to mow and couldnt complete due to electric line, hanging from poles and touching ground. Found owner, Arthur Hunter, of Lowell and met with him to discuss problems. He had not been producing well due to someone telling him he had to have dike to produce. Good IDs on tank, no dike, well produces with jack - small gas leak at well that needs fixed. He said new owner of bar might be buying well, that he would have electric up in next month and gas leak fixed - will start producing well immediately if he is allowed. Owner is 80 years old, ODNR, when called to inform of results of search, said they would probably try and buy well and plug.
1277211476 2010-06-21 Production Wells No Violations Inspection this day found well producing with jack, new gas meter and current chart in box, showing gas producttion being sold. No IDs on tank, owner still is not bonded and insured as required to produce this well. Owner of land is ODNR - e-mailed them telling of the situation and asking how they would like to proceed. Well needs violation. 6/23 - Talked to Brian. He is not bonded and insured - told him he must do that and said he would - will keep me posted.

They say he will need bond for $10,000 as now owns 2 wells. Talked to Brian this day to see how coming along with bonding/insurance. Says he is almost done - has the $1 million insurance and information at the insurance office for bonding. Says hopeful that will have it tomorrow by deadline. Brian reports he is not selling gas, just charting it to see if feasible to sell gas.

1476519971 2010-01-26 Production Wells No Violations Well with jack now producing, electric wire put back up on poles, valves open on jack. Still need correct Ids on tank, ODNR reports Brian Wagner is now owner, has been working on well and will continue to produce it. They are concerned with their property which the well lies on, floods - will they be liable? Jack is 10 up on stand - not sure if or how often jack will get wet. Have call in to Brian Wagner concerning his bonding and insurance.

Still no proof of insurance or bond. 12/7/10 - Brian Wagner now selling gas from well thru Deer Oil. 6/21/11 - RESOLVED for bond and insurance. Still needs to post IDs. Have called many times to reques IDs.

2080588567 2010-12-07 Production Wells No Violations Inspected well this day and found current chart in gas meter - Brian Wagner, owner, had told me he wasnt selling gas but it appears he is. Called Gordy Deer, who owns the lease across the road, and talked to pumper - he says he is changing the chart on this well, that Brian IS selling gas thru them. Well is producing with jack, still no correct IDs, does have good dike. Sup. Stonerock has submitted Orders to Rick Simmers to further process for violations. Brian still not bonded, ODNR is landowner.
