0559619000 2018-09-18 Production Wells No Violations On this day, I am conducting a compliance notice follow-up inspection in regard to a pollution and contamination incident on 8/28/2018. ODNR Emergency Response Nathan Lane has been in contact with OEPA, and they have deemed the issue resolved. It will be noted that the pumping unit is currently operating during the inspection. Mr. Tom Morrison, Frederick Petroleum LLC (owner #6009), was onsite and indicated that he was aware that his liability insurance has lapsed and that he is in the process of acquiring new insurance. I informed him that the well will need registered, he will have to maintain liability insurance, new identification will need placed on the tank, and annual production will need to be reported. Pictures were taken.

Tom Morrison interested in acquiring and producing, Cora Marshall says her heirs did have the mineral royalty for this well and surrounding Ohio Power land.
1535719240 2018-08-31 Production Wells No Violations On this day, I am conducting a follow-up inspection in regards to a pollution and contamination issue from 8/28/2018. The well is setup to be produced by means of a pumping unit with an electric motor, tubing, and rods. The casing is venting to atmosphere, and the tubing is connected to a plastic flowline with the production valve open. Tom Morrison has indicated that the well was never officially registered to him, and that he hasnt operated this well recently. There are (2) 50 barrel steel tanks that sit near a dry creek bed, which terminates into an unnamed drainage creek along Cats Creek Road (CR 76) and Highland Ridge Road (CR 8), that ultimately feeds into Duck Creek. Neither tank is within a dike, and the identification needs updated. The incident reported was estimated at 5 barrels of fluid released from the tank. Remediation efforts are being conducted by using straw, diapers, booms, and constructing (2) syphon dams within the drainage creek. The first dam is approximately 250 feet downstream from the incident, and the second dam is approximately 0.5 mile downstream along Highland Ridge Road. I spoke with Tom Morrison onsite, during my inspection. He indicated that he initially had water trucks flush 2 loads (160 barrels) of fresh water downstream, while an additional vac truck removed 3 loads (240 barrels) of water and product. He is currently using diapers and booms to soak up any additional product on the surface of the stream. Multiple pictures were taken.
1558528857 2018-08-28 Production Wells Violations Noted I was contacted today by Scott King from the emergency response team to inform me of a spill at this address. When I arrived on location emergency response inspector Nathan Lane was already on site. He has tracked the oil to the creek next to the well. The well is coming up under historic owner in the system but there is a faded identification sticker on the tank and previous reports on the well stating that Tom Morrison from Frederick Petroleum LLC is the owner of this well. Tom came out to the location and he will be getting the oil cleaned up from the creek. There is an estimated 5 barrels released at the most. The plan at the time is to have two water trucks come out and flush the contamination all to one spot then soak it up from there. Tom was trying to line up equipment to get this done.

This well sits on land owned by Jeffery L. Devol, Bob Howard, Cora Marshalls brother or uncle, last produced wells. Tom Morrison is working with Cora to acquire the 3 wells pumped by Mr. Howard 25 years ago.
