The GPU unit had been hooked back up and was back in production. The earthen dike had been put back around the GPU unit. The contaminated area from the fire had been cleaned and new limestone installed around the inside of the earthen dike area.

The production equipment included two steel 210 barrel tanks, a steel 100 barrel tank, a GPU unit, and a gas combustor unit. The tanks were inside a three-foot, metal containment area and the GPU unit was inside an earthen dike area. The production report was up to date and proper identification was present.

1541633096 2018-11-07 Production Wells No Violations Received a phone call from Bill Aronhalt at Northwood Energy. He stated there was a fire at the wellhead of this well. Upon my arrival on location, accompanied by ONDR Emergency Operations Coordinator Jason Blocker, I witnessed that the well was never on fire, but the GPU unit exploded. The explosion caught some of the grass on fire on top of the earthen dike. The fire department put the fire out with water. They will scrape the contaminated dirt up after they remove all the damaged equipment from inside the earthen dike. The well was equipped 13.375 inch diameter casing, 9.625 inch diameter casing, 5.5 inch diameter casing, 2.825 inch diameter tubing, rods, and a 640 pumping unit ran with a natural gas motor. The production equipment included two steel 210 barrel tanks, a steel 100 barrel tank, a GPU unit, and a gas combustor unit. The GPU unit was inside an earthen dike and the tanks were on a plastic liner with 3 foot metal walls. The production report was up to date and proper ident ification was present.
