0374253051 2015-02-06 Production Wells No Violations On the date of inspection I was accompanied by ODNR Inspector Randy Akins. I found that the well is equipped with a pumping unit, rods, 2.375 inch tubing, 4.5 inch casing, and 8.625 inch casing. All valves are in open position. Production equipment consists of a 210bbl tank, horizontal separator, and a drip within an earthen dike. Production reporting is up to date.

1156041070 2015-09-10 Plug / Plug Back No Violations Artex Oil is plugging this well to surface. Bishop Well Service has a rig on site setting plugs thru 2 inch tubing. I arrived to set the 3 rd. plug which is a 300 ft. plug at 3123 to 2823 ft. with 75 sacks of class A cement with 2% gel. Formation Cementing is the cement contractor on site to plug the well. Set next plug # 4 at 1156 to 956 ft. cemented with 54 sacks and the last plug to surface at 200 ft. to 0 ft. with 58 sacks. Circulated cement to surface where it remained after job was complete. Did not see any bubbles or leaks after job was finished.
1441282518 2015-09-02 Plug / Plug Back No Violations Upon arrival on location I found the rig ready to set the bottom plug at 7183 feet. They pulled the 2 inch tubing out of the well. They then ran in to tag plug and found that the formation had taken the plug will reset tomorrow.
1441369704 2015-09-04 Plug / Plug Back No Violations Upon arrival on location I found the rig crew ready to set the bottom plug again. The formation has been taking the cement and when the top was tag it show that 15 feet of perforation was still uncover. Established circulation and pumped 35 sacks of class A cement into the wellbore. Shut in for the weekend.
1441807585 2015-09-09 Plug / Plug Back No Violations Upon arrival on location I found that the 2 3/8 tubing was stuck in the 4 1/2 inch casing. The 2 3/8 inch tubing was shot off at 3197 feet pipe would not come free. Will shoot again at 3128 feet, and the tubing came free. We will pull the 2 inch tubing out of the wellbore and then rip the 4 1/2 inch casing.
1446470744 2015-11-02 Final Restoration No Violations On the day of my inspection I found that the final restoration has been completed. The site passed final restoration according to ORC 1509.072 (B).

2058850657 2015-09-03 Plug / Plug Back No Violations Upon arrival on location I found the rig crew ready to set the bottom plug again. The plug was set at 7120 feet to 6720 feet. They will pull the 2 inch tubing out of the well and check for the plug top.
