0818630532 2017-08-16 Production Wells No Violations On the day of my inspection I was accompanied by Inspector Tom Thomas, please refer to his inspection report.
1655664793 2018-07-18 Production Wells No Violations Upon arrival on location, I witnessed the well was equipped 4.5 inch diameter casing, 2.375 inch diameter tubing, and a plunger lift system. The valves were open on the casing and on the tubing. Production equipment included a steel 210 barrel tank, a horizonal separator, and a chart recorder with a 2 by 1.250 inch orifice plate within an earthen dike. Domestic gas was being supplied off the separator. The production report was up to date and proper identification was present.
2101280062 2017-08-16 Production Wells No Violations On the day of my inspection I was accompanied by ODNR Inspector Randy Akins, we found the well equipped with 8 5/8 inch diameter casing 4 1/2 inch diameter casing, and 2 3/8 inch diameter tubing, operated by a plunger lift system. The production equipment is made up of one 210 bbl. steel tank, one recording gauge, and one horizontal separator. The production reporting is up to date, and the tank is properly identified. A minor leak was detected at the wellhead and Artex was contacted.
