| 1559655380 | 2019-06-04 | Production Wells | No Violations | Upon arrival on location, I witnessed the well was equipped 8.625 inch diameter casing, 4.5 inch diameter casing, 2.375 inch diameter tubing, and a plunger lift system. The valves on the tubing and on the casing were open. Production equipment included a steel 210 barrel tank, a horizonal separator, and a chart recorder with a 2 by 1 inch orifice plate. Domestic gas was being supplied off the separator. The 210 barrel tank had an earthen dike around it. The production report was up to date and proper identification was present. |

The production reporting is up to date, the tank is properly identified, and there are no violations at this time. |
