0315105889 2016-04-06 Production Wells No Violations On the day of my inspection I found a 4 X 2 inch flanged wellhead operated by a plunger lift system. The well has been changed from tubing and rods operated by a pumping unit, to a plunger lift system. The tank battery is made up of one 100 bbl. tank, a horizontal separator, and a recording gauge, all in an earthen dike. The tank is properly identified, the production reporting is up to date and there are no violations at this time.

0693034804 2018-08-20 Production Wells No Violations Upon arrival on location, I witnessed the well was equipped with 4.5 inch diameter casing, 2.375 inch diameter tubing, and a plunger lift system. The valves were open on the tubing and on the casing. Production equipment included a steel 210barrel tank, a horizonal separator, and chart recorder with a 2 by 1 inch orifice plate within an earthen dike. Domestic gas was being supplied off the separator. The production report was up to date and proper identification was present.
1382968637 2014-09-17 Production Wells No Violations On the date of inspection I witnessed that the well was equipped with a pumping unit with an electric motor. The well had 4.5 inch casing, 2.375 inch tubing, and rods. Production records are up to date.
1809608087 2017-07-17 Production Wells No Violations This is a follow up inspection to cleanup needed in the dike area. A mist of oil had covered a part of dike area. On inspection of the area today it was discovered that some new oil is in the dike area. I placed a call to Joe Snyder with Artex to inform him of what was found and that it needs to be cleaned up. Joe then returned my call and stated that the pumper for the well would meet me on location to take care of the issue. I met with Tom Simmerman with Artex, and discussed what needed to be done. Once the cleanup is complete Joe with Artex to call and I will reinspect.
1909732255 2017-07-28 Production Wells No Violations Upon arrival on location I found that the cleanup has been completed. The tank has been replaced with one 210 bbl. steel tank. New gravel has been place in the dike area.
