0496865471 2018-09-14 Production Wells No Violations Upon arrival on location, I witnessed the well was equipped with 8.625 inch diameter casing, 4.5 inch diameter casing, 1.5 inch diameter tubing, and a plunger lift system. The valves on the tubing and on the casing were open. Domestic gas was being supplied off the 4.5 inch casing. Production equipment included a steel 210 barrel tank, a horizonal separator, and a chart recorder with a 2 by .250 inch orifice plate within an earthen dike. The production report was up to date and proper identification was present.

1510601846 2017-11-13 Production Wells No Violations On the date of my inspection, I found an overgrown lease road leading to a wellhead equipped with a plunger lift system with motor valve, 1.5 inch tubing, and 4.5 inch casing. There is an identification tag on the wellhead. There is a 1 inch poly line tied into the casing with a regulator in between. The valve to the poly line appears partially open. There is a 2.375 inch line coming out of the casing but not tied into anything. The 2.375 inch production line is tied into the tubing. A valve on the line before it goes into the ground appears to be closed. A pressure gauge on the production line reads 300 pounds per square inch. The production equipment includes a horizontal separator with a chart box housing missing the meter. There is an orifice plate in the sales line. There is one 210 barrel tank with proper identification, a drip, and a concrete brine vault within a partial earthen berm. There is a 1 inch poly line that is tied into the separator that runs towards a barn nearby. There is a bull plug in the heater tube on the 210 barrel tank and some oil staining around the bottom of the tank between the access flange and heater tube. Production records are current.
1550690583 2019-02-20 Production Wells No Violations Upon arrival on location, with the ground covered with snow, I witnessed the well was equipped with 8.625 inch diameter casing, 4.5 inch diameter casing, and 1.5 inch diameter tubing. A swab rig was recently at the well swabbing the tubing. They only had a pressure gauge on top of the tubing. The valve on the tubing was closed and the valve on the casing was open. Domestic gas was being supplied off the 4.5 inch casing. The flow line from the well had not been hooked back up. Production equipment included a steel 210 barrel tank, a horizonal separator, and a chart recorder with a 2 by .250 inch orifice plate within an earthen dike. The production report was up to date and proper identification was present. This inspection was requested from Denise McGiniss, the land owner. She is having trouble with moisture and pressure, since the well has been swabbed. I told her that she needed to contact the well tender about these issues. I placed a phone call to the well owner about this issue. He stated that the well needs swabbed again.
