1141658263 2006-03-01 Production Wells No Violations Met w/Mrs. Mandator, new ID sign posted, exempt domestic well. She is in the process of filing the paperwork to change the well from her father to herself. Information follows: I met with the Landowner, Susan Oaklief Mandator, on February 3, 2006 for questions and information on an Industrial Mineral permit issue. During our meeting Mrs. Mandator mentioned the well and that the package she received from Inspector Grimm had no cover letter, explanation, information, directions nor were any voice mail or telephone communications recieved from Inspector Grimm concerning the package. Mrs. Mandator was under the impression from the package that she would have to purchase a $5,000.00 surety bond and file for a New Well Owner status. During the meeting I called Grimms cell phone and was directed to the voice mail, since Mrs. Mandator was in need of immediate information I called Supervisor Cheslock for the information and informed him of the situation and that Mrs. Mandator asked me for her assistance and that she would like copies of all reports and information on the matter. I advised I would contact Grimm for copies of his reports and forward them to her. 3/3/06 8:30am, call to Sherry Young; update Sherry on the progress. 3/2/06 8:36am email request to Grimm for all copies of reports and inforamtion needed.
1141760356 2005-11-17 Production Wells No Violations Arrived at 10641 Lick Run Rd. Wanted to talk with landowner concerning bonding requirements on a well located at 10641 lick run rd. Nobody home this date. No well inspection was made on this date. Want to inspect well with landowner so I know that myself, Sherry Young and the landowner are are talking about the same well. The farm was transferred to Susan Mandator on 06-12-1989 ( vol 630 -239 ) She has 77 acres on west side of co rd 18 and 40 acres on east side of co rd 18, I want to make sure what well needs to be bonded if there is another well on farm. Called Susan Mandator ( 740-498-6960) this date and left a message for her to call me. If she does not return my call I will try again. 11- 28-2005 called Susan Mandator no answer, will mail ( requirements to obtain an owners number and bonding requirements )
