The operator set the 20 casing to 522 RKB. The conductor casing had a grade of H-40, 94 lbs. per ft., and averaged 44 in length. Halliburton established circulation with 5 bbls of fresh water. The operator pumped 50 bbls of 6% gel spacer followed by 420 sacks of Bondcem System cement. The cement was displaced to 475 using 179 bbls of fresh water. I witnessed approximately 52 bbls of good cement returned to surface. Cement was static at surface after 20 minutes. Cement was sampled and weighed three separate times; at the beginning, middle, and end of the operation.

1188359609 2014-11-18 Preliminary Restoration No Violations I was on location for a Preliminary Restoration. During my inspection, I found all of the drill equipment removed from the location. The site was graded. The berms around the well site were in stable condition and vegetated. The well was equipped with a production tree. The tank battery consisted of five, 400 barrel production tanks and a 500 barrel production tank. A steel dike surrounded the tanks. There was a compressor and emergency flare. No pits were used to drill the well. All debris was removed. Drainage and erosion controls were in place and working properly. The lease road was in great condition with two locked gates.

The pad is fenced and all man doors are locked. The #1H is not in production. The well is equipped with a christmas tree valve assembly and partially rapped in insulation. There is a steel dike around the storage tanks.

The production equipment was in good repair. The proper identification signs are post at the well pad and at the entrance. The lease access was stable. There was a locked gate at the entrance. No violations observed.
1606528576 2013-09-09 Drill / Deepen / Reopen No Violations I received a call from Koreen Burrows, BP Field Environmental Advisor. Koreen stated that all of the grass on the berms surrounding the well pad was killed by a company contracted to kill weeds at all of BPs well locations. Koreen stated the weed killer lasts up to 6 to 8 months. BP plans to reseed the berms by the Spring of 2014. My inspection found all of the grass on the berms dead. The soil in the berms appeared stable. I took photos of the berms.

The production equipment was in good repair. The proper identification signs are post at the well pad and at the entrance. The lease access was stable with a locked gate. 1720113591 2015-08-10 Production Wells No Violations I was on location for a status check. During my inspection, I found the well in production. The wellhead is equipped with a christmas tree. An insulated tarp is wrapped around the wellhead and christmas tree. The tank battery consists of five (5), 400 barrel production tanks, a 500 barrel tank, a heat, treat separator, and a vapor recovery unit. The six (6) tanks are positioned in a steel, lined dike with pea gravel. All of the load lines are encased in locked, plastic boxes. An identification sign is posted at the entrance. The lease road is in stable condition.

I called and spoke with Zane Oshurac from Halcon. I instructed Mr. Oshurac to secure all man doors with locks and post an identification sign at the well.

1883133202 2013-06-23 Drill / Deepen / Reopen No Violations I witnessed Halliburton cement the surface casing for BP. Nabors M07 drilled a 17.5 Hole #2 to 2,988 RKB (Kelly) and set 13.375 surface casing to 2,980 RKB. The casing was 61 lbs/ft and a grade of J-55. Halliburton established circulation with 5 bbls of fresh water. The operator pumped 50 bbls of 6% Gel Spacer followed by 1,270 sacks of Bondcem System cement. When Halliburton reached 330 bbls during the displacement out of 440 bbls; the surface casing began to lift out of the hole. Nabors M07 used the top drive to push the casing down, then held it in place. Halliburton slowed down the displacement pump rate due to the casing lifting. After 357 bbls of displacement, cementing operations was shut down due to pressure climbing to high at the pump truck. I witnessed 6 bbls of good cement returned to surface. Halliburton bled off pressure to the pump truck and the floats held static for 5 minutes. Approximately 641 of cement was left in the casing. After 20 minutes, cement was static a surface. Cement was sampled and weighed three separate times; at the beginning, middle, and end of the operation. All cement sample met density requirements.
1992601560 2018-01-04 Production Wells No Violations I conducted an inspection on this well site with DOGRM inspector John Fleming. At the time of this inspection, the valves on the annulus were open. Production records for this well were current. The production equipment was in good repair. The proper identification signs are post at the well pad and at the entrance. The lease access was stable with a locked gate at the entrance. No violations observed.
2078702450 2013-06-07 Not Drilled No Violations I attended a pre-spud meeting lead by BP. The meeting included an updated casing program that was approved by Columbus and an estimated timeline for the well from start to finish. I walked the entire well pad; drainage and erosion controls were in place and working properly.
