0085092246 2015-07-08 Production Wells No Violations I was on location for a follow-up inspection in reference to compliance notice no. 1775451049. During my inspection, I found the dike is still full of water with an oily sheen. Oil soaked absorbent pads are floating on the water in the dike. I took two high level Quantab readings of the water inside the dike. After 5 minutes, the results of the readings were 4.0 and 4.4 which equates to 793 to 939 ppm chlorides. The trench through the dike in the southeast corner has not been repaired. The 1 inch, black, poly tubing is still lying in the water inside the dike. A pipe rack next to the well with two (2) rods has not been removed. I called Bob Barr at 740-260-2154 and left a message with for a return call concerning the tank battery and the well. 0522942436 2012-10-03 Production Wells No Violations My follow-up NOV inspection found all of the equipment removed from the tank battery and all of the contaminated gravel, soil, and weeds removed. Big Sky was working on adding clean top soil in the tank battery prior to adding clean gravel.
0625871869 2012-10-01 Production Wells No Violations I conducted a follow-up compliance notice inspection on the progress on the oil spill clean-up. My inspection found Bob Barr Jr., Jamie Campbell (field supervisor), and crew had unloaded a pump jack to be put installed at the wellhead. Big Sky was working on removing the two, 100 barrel oil storage tanks and the 50 barrel brine tank in order to remove all the contaminated gravel and soil. I will continue follow-up inspections throughout the week until clean-up is complete.
0644708862 2012-09-06 Production Wells No Violations I conducted a follow-up compliance notice inspection. My inspection found contaminated with oil and brine throughout the tank battery. There was oil and brine pooled next to the separator, a sump pit next to the 100 barrel oil storage tank, oil stained gravel and weeds, and oil/brine soaked pads thoughtout the tank battery. I found two pools of oil covered by straw located approximately 30 west of the well head. According to Larry Criddle Jr., the landowners son, a pine tree located 30 east of the well head was dead from the well spraying oil and brine when Big Sky was swabbed on it in 2010. I performed two chloride tests using Quantab Titrators in front of the separator and at the sump pit in next to the oil storage tank. The chloride level at the separator read 1,063 ppm Cl and next to the oil storage tank read 2,406 ppm Cl. I researched our database and found ODNR Inspector Steve Ochs inspected and documented a violation for contamination on 3/20/2012. He wrote a follow-up report on 6/5/2012 stating the clean-up was uncomplete. I called Bob Barr at 740-624-0434 and told him to clean up all of the contaminated gravel, weeds, soil, and debris immediately. I reminded him that ODNR Inspector Steve Ochs had called him in March on 3/20/2012 and had told him to clean-up up the contaminated oil, soil, and debris. Bob told me, he would get someone out to this location by the end of next week (9/14/2012) to clean the site. This well shares a tank battery and lease road with permit number 2-2502. I will conducted weekly inspection until all contamination is cleaned up.
0831589567 2012-06-05 Production Wells No Violations My follow-up inspection of this well site found the oil clean-up incomplete. Free oil was found on the ground adjacent to the wellhead and inside the tank battery dike.
0916537710 2015-05-06 Production Wells No Violations I was on location for a follow-up inspection in reference to compliance notice no. 1775451049. During my inspection, I observed that the diked tank battery is still full of water with approximately 5 gallons of oil near the northeast corner. I saw 7 to 8 absorbent pads lying in the water inside the dike. I took a high level Quantab reading of the water inside the dike. After 5 minutes, the result of the reading was 6.8 which equates to 2,553 ppm chlorides. I found the trench in the southeast corner of the dike is not repaired. The water outside the dike on the south side still has a sheen. I called and spoke with Don Gooseman, field supervisor for Big Sky. Mr. Gooseman stated the tank battery is on a list to be addressed. I instructed Mr. Gooseman that the water and oil inside the dike and all impacted soil needed removed. Also, I told Mr. Gooseman to fill in the trench. Next, I called and left a message with Bob Barr to return my call. A follow-up inspection will be conducted by 5/17/2015. Additional photos were taken and available upon request.

Immediately adjacent to the wellhead there was 10 ft. by 20 ft. area of soil contamination and oil puddles. There was oil on standing water inside the tank battery dike. There was no off-site impact. Called Bob Bar, told him to clean up oil and contaminated soil and debris.

1353936543 2012-11-26 Production Wells No Violations My follow-up inspection found no identification at the pumping unit, shut in, and no flow lines connect to the well head. The two, 100 barrel oil/brine storage tanks were connected with 2 pipe and a tee in the center. The tee was open with no plug.
1394470996 2014-02-28 Production Wells No Violations I checked this well to determine if the company is in compliance with chiefs order # 2014-43 . I found the tank battery is equipped with 2 100 bbl steel tanks and 1 separator . The gas meter chart had the date of 2-18-2014 marked on it . The marks on the chart for production was not clear if the well was producing or not at this time . The 100 bbl tank to the east was 3/4 full of fluid and the 100 bbl tank to the west was 1/2 full of fluid . The valves from the well to the separator and from The separator to the sales line were open . At the well location I found the well is equipped with a pumping unit . The pumping unit was not operating at the time of my inspection . The valve from the well to the flow line was open . Upon the completion of my inspection I could not definitively determine if the well was or was not producing . NOTE : This well shares a common tank battery with P# 2502 .

During my inspection, I observed that the diked tank battery is still full of water with approximately 5 gallons of oil near the east end and under the ladder for the tanks. I saw 7 to 8 absorbent pads lying in the water inside the dike. I took a high level Quantab reading of the water inside the dike. After 5 minutes, the result of the reading was 7 which equates to 2,792 ppm chlorides. Coiled, poly 1 inch tubing is still lying in the water and oil. I found the trench in the southeast corner of the dike still not repaired. The water outside the dike on the south side still has a sheen. I called and left a message with Bob Barr to return my call concerning the condition of the tank battery. Photos were taken and available upon request.

1549638926 2019-02-08 Production Wells No Violations I conducted a compliance notice follow-up inspection for compliance notice 1775451049, regarding the removal of the standing water inside the tank battery, cleanup around the wellhead, from residual oil. At the time of my inspection, the well was equipped with a pumping unit and not in production. A large tree had fallen onto the pumping unit. The oil staining around the wellhead looked to be degraded and no active leaks were observed at the well, during this inspection. The valves on the well and flow line were closed. The diked tank battery still had standing water within the dike and at the time of this inspection, was covered with ice. Identification posted at the tank battery was missing the company information. Along the west side of the lease access were old rods from the pumping unit. I contacted Big Sky Energy, Inc. and left a message for owner, Robert Barr, regarding the removal of the tree from the well, removal of the previously impacted soil at the well, cleaning up any debris on the site location and amending the identification. The standing water within the diked tank battery, also needs removed.
1775451049 2014-04-14 Production Wells Violations Noted I was on location for a status check inspection. During my inspection, I found the well is equipped with a pumpjack. The tank battery consists of two (2), 100 barrel production tanks and a mechanical separator. I observed that the diked tank battery was full of water with approximately 5 gallons of oil near the northeast corner. There were oil stains on the side of the 100 barrel production tank furthest east. I noticed a section of 1 inch poly flow line coiled up and lying in the water inside the dike. The oil and water inside the dike lead to a pool of water through a trench in the southeast corner two (2) foot south of the dike. There is a sheen on top of the water. I research our RBDMS database and found Big Sky Energy did not file annual production for 2013. I called Don Gooseman, field supervisor for Big Sky. I instructed Mr. Gooseman that the water and all impacted soil/gravel inside the dike and the pool of water on the south side needed to be removed immediately. Mr. Gooseman stated the companys water truck was not in operation. Also, Mr. Gooseman stated he would contact Bob Barr, owner of Big Sky Energy about getting a water truck. Next, I called and left a message with Mr. Barr to return my call concerning the water and oil in the dike. A compliance notice for failure to keep the dike free of water or oil will be issued. A follow-up inspection will be conducted by 4/24/2015.
1794065430 2012-09-17 Production Wells No Violations I conducted a follow-up compliance notice inspection status check on the progress of the oil leak clean-up that was found by ODNR Inspector Steve Ochs on 3/20/2012. A follow-up inspection was performed by Steve on 6/5/2012 and he found the tank battery and well head still contaminated by oil. My inspection found some of the overgrown weeds cut down, but no progress on the clean-up. I was called by the well tender and was told clean-up would start either on 9/17/2012 or 9/18/2012. I will continue follow-up inspections.
