The well owner/operator shall immediately submit a written plugging schedule to the inspectors office at 3575 Forest Lake Drive, Suite 150, Uniontown, Ohio 44685 and the well shall be plugged within 45 days.  The well owner/operator will be notified if the plugging schedule is approved or denied.
1349879244 2012-10-09 Production Wells No Violations I conducted a production well inspection as a request from the landowner. My inspection found the well was a pumping unit that was covered by overgrown weeds, grass, vines, and small trees. I was unable to locate the tank battery or lease road. I researched our database and found compliance violations written by ODNR Inspector Steve Ochs on 12/13/2011 and 4/5/2012. Also, our database shows no production since 1998. I contacted and emailed Jamie Campbell, field supervisor for Big Sky, to inform him the violations need to be fixed, start producing, or file for a plugging permit. I will continue follow-up inspection.

2088209382 2018-03-12 Production Wells No Violations I conducted a compliance notice follow-up for compliance notice 2137643593. There have been no changes to the pumping unit and it was still incapable of production. The valves on the annulus and flow lines were closed. The well was still very overgrown with shrubs and other vegetation. The tank battery was almost completely hidden by overgrown vegetation and consists of one 210 BBL tank and a concrete brine tank. The 210 BBL tank was corroded and full of holes. The buried concrete vault is covered in leaf litter and tree branches. There was a dike surrounding the tank battery with trees growing in the dike walls. There was no identification sign posted. A plugging permit has not been obtained for this well.
