0533512860 2018-03-14 Production Wells No Violations I conducted a compliance notice follow-up for compliance notice 1707876801. At the time of my inspection, the well was equipped with a pumpjack and the annulus and flow line valves were closed. No remediation has been done to the oil and brine impacted area in front of the wellhead. Removal of the impacted soil in front of the wellhead needs done. The tank battery location consisted of two 100 BBL tanks and a mechanical separator. Valves on the separator were open and the production chart inside of the separator has overprinting. Tank battery was diked but the signage on location needs to be updated to reflect owner information.
1158920958 2013-05-23 Production Wells Violations Noted My inspection found the well not in production. The well was equipped with a pumping unit. There was oil stained soil around the wellhead. All the valves indicated the wellhead was not shut-in. The tank battery contained one, 100 barrel production tank, one, 30 barrel brine tank, and a separator. The diked tank battery was covered with oil stained soil and grass. There was a 1 poly black flow line coming from the separator and stretched across and on top of lease road that appeared to go to a residence property east of the tank battery. There was no identification posted. The lease road was in stable condition. I researched our database for production records. I found zero production from 2007 to 2009 and no reported production for 2010 through 2012. I called Big Sky at 740-624-0434 and left a message to clean-up the oil stained soil and grass inside the tank battery and wellhead. Also, I said I would be mailing a compliance notice to produce or plug the well and no identification. All violation need to be corrected by 7/1/2013. I took photos of the violations.
1252770380 2014-09-15 Administrative Inspection No Violations I conducted an administrative follow-up inspection. I called Bobby Barr Jr. concerning the proof of ownership and submitting annual production records. Bobby stated the company was working on finding the proper paperwork for ownership of the well. Also, he stated the company submitted the annual production statement under the wrong well. Big Sky was working on correcting the mistake. A follow-up inspection would continue until both issues are corrected.

1381321949 2013-10-09 Production Wells No Violations My inspection found the proper identification posted on the production tank per my request. I noticed no further violations at this time.
1409748505 2014-09-03 Production Wells No Violations I was on location for a status check. I found during my inspection the well in production with a pumping unit. The tank battery consisted of a 100 barrel production tank and a separator. A dike surrounded the tank battery. A Big Sky identification sign was posted on the production tank. There was a plug on the load line. There was a production chart in the separator dated 8/27. The lease road was in stable condition. According to our database, B-J of Ohio is listed as the well owner; however, Big Sky was submitting annual production up to 2009. I called Bobby Barr, Jr. concerning the ownership of the well. I told Bobby to submit the proper transfer of ownership paperwork and to submit the annual production statements for the well. I told Bobby a follow-up inspection would be performed by Sept. 12, 2014.
1466173962 2016-06-02 Production Wells No Violations Follow-up to Compliance Notice #1707876801 issued on 5/2/2016 due to pollution and contamination issues found at this wellhead. My inspection found the valves on the tubing and casing open at the wellhead. An inspection of the affected area in front of the wellhead found that none of the remedial activities required by the prior Compliance Notice have been undertaken to address the pollution and contamination issues. The meter chart indicates pressure at the separator and the inflow and outflow valves are open. The domestic tap on the pop-off valve is open and I could hear liquid being dumped to the 100 barrel tank during my inspection. I photographed a crude oil purchase ticket left by Ergon Trucking in the separator that indicated a shipment of 8 and 8 of crude oil from the 100 barrel tank on 5/3/2016. Big Sky Energy Incorporated continues to be in violation of Chapter 1501:9-1-07 of the Ohio Administrative Code and Chapter 1509.22(A) of the Ohio Revised Code.

-off valve has two (2) open ball valves, a Big Joe regulator and field regulator. The production chart inside of the separator has overprinting. An identification sign is posted on the blue 100 bbl. storage tank. Two (2) 100 bbl. storage tanks, a 25 bbl. storage tank, a separator and ladder have been added to the diked tank battery.

Big Sky still needs to: Scrape up and remove all of the affected material down to good soil or clay. Dispose of the affected material at an approved landfill. Remove the three (3) storage tanks, separator and ladder stored on site.

1707876801 2016-04-18 Production Wells Violations Noted I conducted this inspection to follow---105 requiring Big Sky Energy Incorporated to suspend all oil and gas operations in the State of Ohio. My inspection found the wellhead equipped with an electric powered pumping unit. The power to the unit is currently off and the tubing and casing valves are open. There is an area measuring 25 X 25 to the north and west of the wellhead that has been grossly impacted by oil and brine during production operations. The wellhead is covered in oil and there are several soaked absorbent pads in the release. It is apparent that no attempt has been made by Big Sky Energy to mitigate the spill and remediate the impacted area. The well produces to a diked tank battery containing a mechanical separator and a 100 barrel production tank. The flow line and sales line valves are open at the separator and the meter chart has been overwriting production data for some time. There is a 1 line plumbed to the pop-off valve on the separator that is supplying domestic gas. The proper ID is posted at the tank battery. Big Sky Energy shall, within one (1) week of the issuance of this notice, scrape up and remove all of the affected material down to good soil or clay and dispose of the affected material at an approved landfill. Big Sky Energy Incorporated has failed to comply with the above orders to suspend oil and gas operations in the State of Ohio and shall immediately cease the production operations of this well as required by the temporary restraining order issued by the Trumbull County Court of Common Pleas, dated 4/11/2016.
1816332700 2013-09-23 Production Wells No Violations My inspection found all of the oil stained grass and soil removed from the tank battery. Also, the pools of oil and water were removed. The proper identification sign had not been installed per my request. I called Well Completion Service; Valerie stated new signs were made and would be installed shortly.
2068176626 2013-09-05 Production Wells No Violations My inspection found the oil stained soil and weeds removed from the tank battery and well; however, there were small pools of oil inside the tank battery. I called Bob Barr, Jr. at 740-624-0434. Bob stated he was still working on cleaning up the tank battery and plans to post the proper identification sign.
