At the time of this inspection, the annulus and flow line valves on the well were open. Identification has been posted at the well. The tank battery consisted of two 100 BBL production tanks, two 50 BBL steel brine tanks. Two heater treater separators were also present and had been installed outside of the diked tank battery. Valves on the separator were open and the chart dated 11/9/17 showed production. The oil impacted soil within the diked battery has not been removed and both heater tubes on the 100 BBL production tanks were actively leaking. Identification signs have been stenciled onto the right door of the separator with the current owner information. The access road was stable. The tank battery was shared with permits #2-2236 and #2-2235.

The well is equipped with a plunger lift assembly. The tank battery consists of a 100 barrel production tank, a 50 barrel brine tank, and two (2), mechanical separators. A dike surrounds the tanks. An identification sign is posted on the separator. A plug is installed on the load line. The lease road is paved and in good condition. The well shares the tank battery with permit numbers 2-2235 and 2-2236. The well was placed into production prior to House Bill 278.

One of the heater tubes on the 100 BBL production tank was still actively leaking. Identification signs have been stenciled onto the right door of the separator with the current owner information. The access road was stable. The tank battery was shared with permits #2-2236 and #2-2235.
1322163831 2016-04-19 UrbanProduction Wells Violations Noted I conducted an inspection on this well. The well is equipped with a plunger lift assembly and the valve on the wellhead was open. The tank battery consists of two, 100 BBL production tanks, two, 50 BBL brine tanks, and two mechanical separators. The last date on the chart was 2/29/16 and showed production. There is pooled oil within the tank battery covering half of the inside of the tank battery. Absorbant pads have been placed on top of the pooled oil. A dike surrounds the tanks. The well shares the tank battery with permits 22236 and 22235. Identification signs are posted on the separators but not the wellhead. The lease road is stable and driveable. The following items need addressed:

1) Clean-up and remove all of the oil in the tank battery and properly dispose of the impacted soil. 2) Add signage to the wellhead. 3) The well needs shut in immediately per the temporary restraining order issued by the Trumbull County Court of Common Pleas, dated 4/11/2016.

1549389416 2019-02-05 UrbanProduction Wells No Violations I conducted a compliance notice follow-up inspection for compliance notice 1322163831, regarding the repair of the leaking production tanks and cleanup of impacted material within the tank battery. At the time of this inspection, the well was equipped with a plunger lift assembly and in production. The tank battery consisted of two 100 BBL production tanks, two 50 BBL steel brine tanks. Two heater treater separators were also present and had been installed outside of the diked tank battery. Valves on the separator were open and the chart on the sales line showed production. The oil impacted soil within the diked battery could not be seen due to the standing frozen water within the battery. The heater tube on the 100 BBL production tank was not visible due to frozen ice around the tank, and I could not see if the heater tube was still leaking. Identification signs have been stenciled onto the right door of the separator with the current owner information and have been installed at the well location. The access road was stable. The tank battery was shared with permits #2-2236 and #2-2235.
