0496994901 2019-03-12 UrbanProduction Wells No Violations I conducted a compliance notice follow-up inspection for compliance notice 1462017995, regarding the cleanup of the oil impacted soil at the base of the 100 BBL tank and the residual oil along the side of the tank, the rebuilding of dike around the tank battery and repairing the leaking 2 inch flow line at the well. Identification also needed to be installed at the tank battery with updated owner and well information. At the time of my inspection, the well was in production and equipped with a plunger lift assembly. The gate valve on the tubing and the 2 inch flow line valve were open. There was oil along the 2 inch flow line below the elbow connection, prior to where the flow line was buried. The residual oil along the south side of the 100 BBL tank was still present and had not been cleaned off of the tank. The area of impact at the base of the tank was stained dark but no fresh oil was present or active leak observed, at the time of this inspection. The 100 BBL tank needs cleaned of the residual oil and the soil at the base of the tank needs removed and properly disposed of. The dike around the tank battery needs rebuilt. The identification still needs updated with current owner information. I contacted Vortex Energy, LLC and spoke with field superintendent, Greg Henderson regarding the cleanup of the oil on the tank and removal of the impacted soil at the base of the tank. I also discussed the reinstallation of the dike and updated identification.
1215781175 2016-04-07 UrbanProduction Wells No Violations I was on location in reference to compliance to Chiefs Order 2016-105. At the time of my inspection, I found the 2 flow line valve entering the separator in the open position. The 2 sales line valve coming off of the separator is in the open position. The two 2 flow line valves at the well are in the open position. There are no additional changes from my previous inspection on 21 March 2016. Photos were taken and available upon request.

The equipment on the site consists of the following:

1462017995 2016-04-28 Production Wells Violations Noted My inspection found the well to be equipped with a plunger lift unit. The valve leading from the wellhead to the flow line is closed. There is a domestic supply plumbed into the 4.5 casing. The tank battery consists of a 100 BBL production tank and a 50 BBL brine tank. The tank was overfilled at one point and the South side of the tank has oil down the side of it. There is a 3 ft by 5 ft oil saturated stone and soil at the base of the tank. The production tank needs wiped down, the oil saturated soil at the base of the tank needs removed and properly disposed of, and the dike around the tank battery needs rebuilt. The heated separator is outside of the tank battery with the proper spacing. The production chart was last changed on 2/29/16. The well was not in production at the time of the inspection. The identification sign is on the tank battery and needs to be updated. The lease road is in stable condition.
