1044804050 2014-01-16 Production Wells No Violations My inspection found the well in production. The well was equipped with a plunger lift unit. The tank battery consisted of a 50 barrel production tank, a concrete brine tank, and separator. There was a plug on the load line. The dike met containment requirements. An identification sign was posted at the separator with the previous owners information. The lease road was in good condition. I called Kleese and told them to post a new identification sign with the correct information.
1553091375 2019-03-20 Production Wells Violations Noted I conducted an inspection on this well. The well was equipped with a plunger lift assembly and not in production, at the time of this inspection. The gate valve on the tubing and the valve on the 2 inch flow line were both closed. There was a domestic line with a regulator plumbed off the 4.5 inch casing with an open valve. I spoke with landowner, Mr. Steve Soroka, who stated that he does not receive gas for domestic use and has not had domestic gas since 2013. Under Kleese Development Associates ownership, production records from 2015-2017 have been submitted as zero. Production records for 2018 were not available, at the time of this inspection. The diked tank battery was comprised of one 50 BBL tank. There was a mechanical separator outside of the diked tank battery with an open valve on the inlet side but a closed valve on the outlet side. The chart present on the separator was a large blot of ink and did not show any current production. No leaks were observed from the well or separator fittings at the tank battery. The identification sign was present on the well and had outdated owner/operator information and did not contain the well information. The lease access was stable. I attempted to contact Kleese Development Associates at the phone number on the posted identification on site. The phone number was a non working number. I contacted Kleese Development Associates at the phone number listed on the state database. The number was up to date and I left a meassage requesting a call back regarding the compliance issues addressed in this report. The well owner shall submit a written plugging schedule to Inspector Shannon Mullins at ODNR Division of Oil and Gas Resources Management, 3575 Forest Lake Drive, Suite 150, Uniontown, OH 44685 within 30 days. The well owner/operator will be notified if the plugging schedule is approved or denied. The Soroka #1 shall be plugged within 180 days. In the interim, the owner must correct the identification sign
