1429805013 2015-04-02 Salt Water Injection Well Violations Noted At 2:42 pm, I received a call from our Uniontown ODNR office about Complaint ID 4989 filed by ODNR Wildlife Officer Marty Cisine for Mr. John Hopkinson at 884 Sodom Hutchings Road. I called Officer Cisine immediately, who stated that Mr. Hopkinson had a pond on his property with a dead turtle, muskrat and fish. In addition to the kill, Officer Cisine stated Mr. Hopkinson expressed concern about an oily smell and film on the pond and that his property was located across the street from the Kleese UIC facility (KDA). I left my residence at 3:20 pm and headed to the location to investigate the complaint.

At that time, ODNR Chief Simmers, ODNR Deputy Chief Worstall, ODNR Deputy Chief Claus and ODNR Northeast Regional Hydrologist Kyle Tharp arrived on scene. I briefed the Chief on my findings from the previous evening and today, and then showed him the oil in the ditch with the pea

1430312743 2015-04-08 Salt Water Injection Well No Violations I arrived on location at 8:10 am in reference to compliance notice no. 1429805013. First, I inspected the four foot culvert along Sodom Hutchings Road. I found that Kurt Kollar of the Ohio EPA and IWC had placed two rows of soft, absorbent booms followed by a hard boom within five feet of the outlet. Upon further inspection, I found no new free product coming from the culvert pipe. I was informed by KDA that Myers Oil Field Service was contracted to work with IWC to continue cleaning the oil spill. I observed Myers and IWC actively cleaning the ditch and wetland area near Sodom Hutchings Road. Impacted weeds were being removed and bagged for proper disposal.

Next, ODNR Inspector Ochs and I instructed KDA to extend the cut-off trench further north by 100 feet to ensure the source was completely stopped. All impacted soil was placed into lined containers. All free product and water was being removed using a water truck.

Once the cut-off trench was completed, Inspector Ochs and I instructed KDA to begin digging next to the north unloading pad.

At 2:30 pm, a meeting was held with Mr. Kollar, Myers, IWC, Hull & Associates, Klienfelder, and KDA to discuss a chain of command and a new strategy to clean-up the oil spill. Myers took the lead for all clean-up operations with IWC assistance. Hull & Associates sectioned off the KDA facility and impacted site into 10 different zones. Hull was tasked to test all impacted water and soil for proper disposal. Klienfelder and KDA were tasked with exposing the septic tanks, demolishing the unloading facility, and removing all storage tanks from the concrete pad.

The company pressured the flow line to 1380 psi and then shut-in the flow line. I monitored the pressure for 15 minutes. There was no loss of pressure. Kurt Kollar of the Ohio EPA, Inspector Ochs and I met with KDA management to establish organization to the overall clean-up, determination of the source and remediation of the oil spill. Lance, KDA field supervisor, was assigned by KDA as the field manager for all operations related to the oil spill.

Mr. Kollar and Inspector Ochs discovered a drainage crock and sump pump covered over with a steel plate located along the dirt drive on the east side of the facility. Condensate and water was found inside the crock. Additional product and water was entering in from small drainage holes in the side of the crock. At 9:30 am, Mr. Kollar put green dye into the crock along with four five-gallon buckets of water to determine where it was draining. We found the green dye pumped into a septic tank located in the center of the storage tank pad.

KDA was excavating up against the east side of the north unloading pad. At the bottom of the 5th cement block, oil and water was seeping out of a crack. Also, KDA was actively removing all fluids and sludge from the septic tanks located below the unloading facility. While cleaning the septic tank furthest to the east, KDA discovered cracks in the wall. Lance decided to finish cleaning the septic tank, fill it with fresh water and perform a dye test to check for a possible leak.

Next, KDA and Klienfelder began to focus on the northeast corner of the storage tank pad at the interface between the dike liner and the unloading facility. KDA created a mini-dam inside the diked area with clean dirt and dumped water with red dye. After approximately 15 to 20 minutes, red dye began to emerge into a trenched pit located 15 feet to the northeast. I noticed the red dye was pouring in from the geotextile liner between the slag and clay soil. Upon further investigation of the dike liner under the gravel

Then, Mr. Kollar, Tom Hill, Inspector Mullins and I had a meeting with KDA management to see if they could recall any possible cause to the spill. KDA management was steadfast that they knew nothing about tanks overflowing. Also, KDA, Klienfelder, the company who consulted on the design the facility, would be heading up the excavation of the entire facility.

Afterwards, Tom Hill, Inspector Mullins and I worked our way back to the UIC facility to continue investigating a source and cause. At the same time, KDA began to excavate a shallow trench next to the unloading pad. While exposing an electrical line, some oil and water began to seep into the trench. As trenching continued; more product and water entered. KDA was reminded that a cut-off trench needed to be dug to isolate the oil spill.

At 6 pm, Supervisor Tom Hill and I went to a Vienna Township Trustee meeting to inform the local public about the spill and progress of the clean-up. The meeting lasted from 6 pm to after 7:30 pm.

Afterwards, Tom Hill and I went back to the KDA UIC facility. KDA was actively digging next to the unloading pad to expose the septic tanks. I informed KDA that cut-off trenches needed to be started.

There was an initial pressure of 470 psi. on the tubing. A pressure of 1140 psi. was placed on the tubing/casing annulus. With the tubing pressure holding at 470 psi., there was a loss of 25 psi. on the annulus observed, during a fifteen--125. There were no leaks or additional issues found.

2055458712 2015-04-12 Salt Water Injection Well No Violations Complaint# 4989, Marty Cisine. I was on site to witness the clean-up from a brine and oil spill at a injection well storage facility. There were brine trucks removing fluid from trenches on east side of storage facility. There were also brine trucks removing fluid from sump holes near the west side of Sodom Hutchings road. The brine trucks would remove fluid in trenches and sump holes as it collected in them. They would transport the fluid collected to 500 bbl frac tanks on north side of storage facility. The company was injecting fluid into five injection wells from the storage facility the entire time I was on site. I spoke with the workers overseeing the injection of fluid, they told me they had to adjust the volume of fluid being injected due to the pressure rising in the injection wells due to the light fluid. When I left the site there was 3000 bbls of fluid in the tanks in the storage facility, 3000 bbls of fluid in six 500 bbl frac tanks on north side of the storage facility and 1000 bbls of heavy brine being stored in two 500 bbl frac tanks on south side of the pump house. The workers told me they were able to inject 4000 bbls of fluid into the injection wells in a 24 hour period at this time depending on the current injection rate and pressures.
